The Scramble Match for example is a hard to work match because something unplanned can happen with the time limit stipulation. Has there been matches where wrestlers messed up trying to pull off last minute pin falls or something?

Table matches have been problematic in the past. Remember where Kurt accidentally bounced D-Von off of like 2-3 tables with suplexes when they were supposed to break?

Wrestlemania 16 springs to mind, nobody knew what was happening there.

None in WWE or WCW that I can think of, just going by the Iron Man matches I can remember.

None of the three Scramble Matches finished on time, just for the record.
The times were 20:04 (ECW), 20:16 (SmackDown) and 17:15 (Raw)

That Hardcore match at Wrestlemania with the whole Hardcore Holly/ Crash Holly confusion? Yeah, I seem to remember a timing issue with that. I can't recall exactly what happened, but it came off as an issue of the time running out before they had a chance to execute the planned finish.

Anything with a time limit.
If you watched the Beat the Clock challenge match between Shelton and Trips, the ref had to look at the clock while he crawled so he could be on time for the finish.

Ladder and cage matches can make wrestlers look stupid as they sometimes just stand at the top of the cage/ladder waiting to be stopped.
Simultaneous battle royal eliminations or pin-falls can be tricky.
I think HHH and the Rock's iron man match went longer than an hour.

Scaffold is absolutely #1. Probably the most common of terrible matches.

Has there been matches where wrestlers messed up trying to pull off last minute pin falls or something? Lots of times in varying scenarios/stipulations. One off the top of my head: Matt Stryker and Kurt Angle apparently ran too long in an Angle Invitational, so Stryker had to tap immediately when the hold went on. Peripherally related is the botched finish where the ending of the match is dependent on someone making a run-in. Papa Shango was too late for Sid vs Hogan at WrestleMania VIII, so Harvey Wippleman had to enter and immediately create a disqualification. This, too:

He shook the ropes at the other side of the ring to make Rico fall.

From what I've heard, just basic triple threat matches are hard to work, because you have more people interacting with each other, and it's hard to do on the fly. I think that's why you see a lot of 1 on 1 action before the other guy comes in, then more 1 on 1.

The Punjabi Prison match.
I can't really see anyone making this match good.

I think that's why you see a lot of 1 on 1 action before the other guy comes in, then more 1 on 1. I think they do a good job of it in WWE for that reason, not overdoing it. They toss in 2 on 1 parts, and one guy fighting both off, but in the end, it's usually the logical steps of either knocking one person out of the running, or one hiding to wait for their spot.
