mine is getting through all 100 levels of the Ancient Cave and beating the damned jelly the real way (didn't know of the cheat at the time) on the actual SNES (ie: no savestates) in Lufia 2

Finally beating Yo Noid! as a kid on the NES...that game was so cheap in the later levels that I broke 2 controllers in frustration

Beating Super Mario Bros without losing a life.
Beating Brian's story mode on Tekken 3 on the hardest difficulty without taking any damage. (still have no idea how I did this and I could never ever do it again in a million years)
Winning the Super Bowl on Tecmo Super Bowl while playing through a season where the only way we were allowed to score was with safeties. I barley squeaked into the playoffs then won every playoff game and the Super Bowl with ease.
My crowning achievement though is beating every single Suikoden game to perfection with little to no help from walkthroughs. By perfection I mean all 108 stars every miscellaneous item in the game like sound sets, recipes, ect, and also unlocking every hidden scene in the games Clives quest, bath house stuff ect.

I beat The End in Metal Gear Solid 3 with a KNIFE. I beat a sniper with a KNIFE. And I mean I chased his old ass down and knifed him to death. I had no ammo at the time, so the knife was all I could do.

I learned the "1up" turtle jump on the stairs trick from Super Mario Bros one day at the store (remember that.... going to a store to play video games).... I soon moved to the other side of the state where one day I was playing Super Mario Bros.... doing the trick... next thing I know there were kids all around in SHOCK. They were calling others over in disbelief as they have never seen the trick before. I had to play again for all the other kids after school to show the trick again.
I was one power glove away from being Lucas Barton from the Wizard... I felt like god.

And no, I can't do it, but One Life Clearing Danmaku games are pretty much the ultimate judge of video game dexterity.

In terms of personal accomplishment?
Trying for years (think it was like 4) to beat the first Zelda game and never coming close. Finally, one day, bought this book that said it could help me do it, deleted my original game and started over. Beat it in two days. Dunno if the book just boosted my confidence or gave me the tips to get more heart containers but what a feeling when I finally won.
The next closest was beating Final Fantasy on the NES. GOD, that game took forever but to finally win, see the final screen... then realize I was stuck in a time loop and the game never ends. That one emotionally broke me for a while. But hey, it was worth it to finally beat the darn thing.

Running through the coaster in L4D2 on Versus after my teammates fell and incapped. I murdered a hunter and jockey back to back hopped up on adrenaline, gunned straight through a spitter, sidestepped a charger and left him fall off and narrowly avoided a smoker at the end somehow. I lucked into finding another adrenaline near the end. They were leading, but we shut them down so we agreed if someone fell and got incapped to just run.
Oh, and doing a million point combo in THPS4. I didn't even own the game, it was my friend's copy and I had only played 2.

Last night I played Team Fortress 2. I was a Pyro in Well and I got 12 kills, 1 Death and I captured every control point.

Zombie Genocider. Yeah, I got THAT one. Just get a truck and start running over zombies. It takes ages but is relatively easy. 7 day survivor, that's where you gotta be real good.
