Mine is a tie between beating Call Of Duty World At War on Veteran and 7 day survivor on Dead Rising....my wife is awesome for letting me play through 14 straight hours on that game.

And no, I can't do it, but One Life Clearing Danmaku games are pretty much the ultimate judge of video game dexterity. Did that once on R-Type Final, though I guess that's not really a danmaku game. Got every ship in the game though. On the other hand, I managed to do a one-life run on Touhou 10, and a one-life, no bomb, on Touhou 8. I was on the zone during those times though, which are pretty rare. Also, managing to beat Rugrats: Search for Reptar in about an hour. Ha ha ha.

My crowning achievement though is beating every single Suikoden game to perfection with little to no help from walkthroughs. By perfection I mean all 108 stars every miscellaneous item in the game like sound sets, recipes, ect, and also unlocking every hidden scene in the games Clives quest, bath house stuff ect. You sir are a god! Or a dirty dirty liar. Trust me It took a very very long time, and usually about 5 or 6 play throughs per game to do it. Usually by the time I get everything to perfection the next game is ready to come out. Been waiting for 6 forever now though. I will admit to using the internet to find some of the bath scenes in three, and one of the mermaids in four. If I didn't love the Suikoden Series so much I would never even attempt it. For some reason though Suikoden is just one of those games that I never tire of right now I'm in the middle of going back and playing through all of them again.

Ok, I just remembered an even bigger achievement of mine.
In NCAA Football 07, I took the San Jose State Spartans, the overall weakest team in the game initially, to the BCS National Championship game against Ohio State and WHOOPED them. I did it in two seasons, and the only game setting I changed was the quarter length.
In these two seasons, the ONLY team to beat me was a #20 ranked California team. They were the first team I face, I've won every game since then. In these two years, I toppled teams like Texas, Oklahoma, USC, Florida State, LSU, Ohio State, Miami, and West Virginia. The key to my success was stopping the run. I focused my defensive play calling on run defense. Win in the trenches, I always say. And it worked. The only teams to come close to beating me were Miami and Hawaii, two teams that, in this game, lived and died by the passing game. I had to go to overtime to beat both of them in the BCS Championship season.
The reason this was a huge accomplishment for me wasn't cause I won the BCS title with the weakest team in the nation. It was because San Jose State is in the WAC, a non-BCS school. I broke the system.
Granted, I was EXTREMELY lucky in getting to the title game. Going into the conference championship weekend, I was #4 in the BCS rankings, and both the #2 and #3 had championship games that day....and both were defeated, springing me to #2. And damn if I didn't feel good. My first 3 opponents of the season were #1 ranked teams. I originally scheduled the preseason top 4 teams as my non-conference opponents, and beat each of the first 3. The fact that I wasn't ranked until week 8 pissed me off, but once ranked, I shot through the ranks each week.
And the kicker? After the season, I, as the coach, got offers from places like Notre Dame, USC, Oklahoma, and even Florida. I turned them all down, opting to get a 6 year extension with SJSU.

Getting my girlfriend hooked on video games. She's even more crazy about them than I am I think. She's played Fallout 3 about 9 times already (including the DLC) and she can't wait for Fallout:New Vegas. She is so hooked that SHE came up with the idea that we get ourselves a PS3 for Christmas because it would be a perfect gift for both of us! She even gets impatient if I don't play something!

Doing 110 Super Jumps in Super Mario RPG for the first time.
That and, one time in an online battle with Super Smash Bros. Brawl, I won a match when, as someone was returning onto the platform, I either was playing as Mr. Game and Watch or as Samus...if the former I did the hammer and got a 9 JUST as the person was about to land, getting the win. If the latter, it was a fully charged cannon shot.

I beat all the X missions in the Trauma Center series, XSing all of them except X5 and X6 in New Blood.
Also, I did a Level 3 game of Super Mario RPG (Mario can only level up to 3, Mallow can only go up to level 4, and the rest of the party stays at their default levels).

Also, the odd thing about Ibara is that it's designed in such a way that No Miss Clearing will actually yield a lower score than INTENTIONALLY killing yourself if you have lives to spare. Mind you, I'm absolutely horrible at Ibara, and don't quite have the money to throw around to be playing Mushihimesama (or its sequel) or Mamoru Has Been Cursed, or Muchi Muchi Pork, or any of the other overpriced Cave-developed won't-run-on-anything-but-a-Japanese-system games. I'd like to be good at games like that, but it's not happening.

Crowning the Witch in Left 4 Dead 1 the first time. Now that I know HOW to do it, I can do it every time (especially to the wandering Witch in L4D2; PS, you can also chainsaw-crown her), but it took me quite a few attempts to get it done.
Beating Rock Band 2 on expert everything except singing. I can now complete every song on expert, I just may not be able to gold star them.
Beating Super Mario Bros 2. It was the first in a long line of video games I've defeated.
Doing Mario Baseball tournaments in my old dorm at the Air Force base. I never lost a game, but MAN some were close... It came down to a rule that I could not have both Daisy (home run MACHINE) and Yoshi (any ball in the gap is an inside the park home run, he's so fast.), nor could I pitch as Waluigi or Donkey Kong. The new Wii version is fun as hell, but no one wants to play it.

I have defeated the Emerald WEAPON, the Ruby WEAPON from FF7, and I think the Warmech from FF1, but that was like ten years ago.

Getting the Crazy Hand Trophy in Super Smash Bros. Melee.
Also, when Super Smash Bros. Brawl came out, I made it to the final two in a GameStop tournament. This included successfully defeating and unlocking both Ness and Marth all by myself.
