How about Tourny Of Death 2. The Bump off the roof that killed Nick Mondo's Career ( And nearly Nick Mondo.) That bump was definitely insane (the tables that were supposed to break the fall of Zandig & Mondo were too close to the building and were thus largely missed), but Mondo intended to retire after TOD2 whether the spot went right or not.

Obviously it pales in comparison to falling off a scaffold through 8 flaming tables covered with thumbtacks but I still cringe at the WM10 ladder match when Razor takes a backdrop to the outside, landing on the concrete floor where the pads had been pulled back.

That screwed up Marek's career big time because of the time he missed and the momentum he lost. He was seen as being an equal to, maybe even better than Tyler Black at the time.
Now Tyler is in FCW and Marek is...well, I've not seen him mentioned at all on the larger indies' cards lately.
You could pretty much take your pick for bumps off the Ultimate X structure too. AJ's had a few vicious ones.

Hah, I love how he falls through the table afterwards. That's just too perfect. I've seen that video before but I never noticed that part. It really sucks that the botch ruined his career though. He deserves a second chance simply because of the insanity of the move he was attempting. The 40 Second mark in that video is pretty damn insane. Also, this one. Holy s***

Some Raw in 1997 before WrestleMania (might have been just barely after Royal Rumble) Faarooq & Savio Vega were wrestling and both standing on the second rope. I think some sort of superplex was planned, but both took a tumble hard to the floor. Simmons landed on his head and Savio was pretty shaken up.

Shane McMahon - any of his big drops (don't care how gimmicked they are, it is still a long way to fall), and the suplexes into/through the glass courtesy of Kurt Angle. Shane really was underrated as a wrestler imo

He has a real job now, he's like a lawyer or something. He's just the type of guy that knew when to get out. Unlike Nick Gage who would've been doing the same things he's been doing until he either OD'd or died from some botch.
