The Rock getting beaten up in a locked coffin and Sgt. Slaughter/other backstage people opening it up and being horrified at what they saw. At first the camera didn't show what Rock looked like, which was actually kind of cool as it let viewers use their imaginations and think that the attacker was real heinous. Unfortunately, they ruined this by the end of the night by showing Rock who just had a little blood on him possibly from an incredibly weak blade job. Didn't Triple H beat on it with a Sledgehammer? I think this was the summer of 99. And Mankind did the same thing to Undertaker 3 years earlier. I believe you're right.

Not sure if any of these were mentioned yet, but.. - Bobby Heenan spraying something into the Ultimate Warrior's face during Warrior's "Posedown" with Rick Rude at the '89 Royal Rumble. Warrior no-sells it, and Rude attacks him from behind. No one ever mentions what that spraying was about. Another one would be Lance Storm showing how bad he was by returning a video tape to the store without rewinding it. WHAT AN ASSHOLE! That could mildly inconvenience me!

In 1995, Duke Droese starting a match against Marty Jannetty as a face, turning heel in the middle of the match, losing, and coming back as a face next week.

There was an episode on ECW where Marcus Cor Von missed his cue to be eliminated, so he jumped over the top rope to eliminate himself. Anybody remember this?

From Memphis, Robert Fuller was feuding with Jeff Jarrett. A video airs of Fuller interviewing an elderly Nick Gulas about how Jerry Jarrett stole his business. I've heard it rumored that Gulas may have believed this was a real expose' and not an angle. It used to be on Youtube, not sure if it's still available.

I know for sure when Jeff Hardy was a jobber in the mid-90s, there was a graphic that listed his name as "Jeff Harvey". Over 10 years before he Mike Adamle said it.

During his stint as a commentator on Velocity, Ernest 'The Cat' Miller would frequently refer to Albert/A-Train as Denzel Washington. He'd just kind of say it as Albert/A-Train made his entrance, like some kind of non-sequitir except he said it every damn week! I'm pretty certain it was never explained and Josh Mathews (who was doing play-by-play) never questioned it.
Please tell me someone remembers this.

After a mix tag team match between The Rock/Lita and Triple H/Trish Stratus, The Rock hits Triple H with a steel chair and the latter fell on Trish and they were like a 69 position. Triple H and Stephanie celebrating their 1st (kayfabe) anniversary and they wanted to invite Austin just so Triple H can attack him in the dark. He ended up beating Vince. Big Bossman beating The Rock to become the #1 contender for Big Show's WWE Title at Armaggedon in 1999.

That weird elimination spot between Bret Hart and Tully Blanchard at the 1988 Survivor Series. It looked like Hart eliminated Blanchard, but Hart's shoulders were on the mat, while Tully's were up.

Jacqueline winning the Cruiserweight title from Chavo Guerrero only to lose it at the next PPV. It came completely out of nowhere and after she lost the title, I don't recall it ever being mentioned again.

I have no idea on the date, but I remember one Nitro that had an entire first hour dedicated to some NWO videos. I think one of them had Nash hanging out at a pool hall with Luger (maybe) and Torrie Wilson. The videos weren't that long, maybe like 5 minutes each, but they kept replaying the same two or three for the entire hour. At the start of the second hour, Nitro starts normally and I don't think any of the announcers even mentioned the videos.

There was a short lived comedy angle in WCW where Norman Smiley was getting beat up frequently and was anxiously awaiting the return of Glacier to consult him for protection.
Once Glacier rolls around he excitedly accepts Norman's offer, swearing Norman doesn't have to worry anymore, but for at least the couple of weeks that this angle existed, Norman would have matches against guys like Bam Bam Bigelow, where Glacier would come out, turn a blind eye to the match and greet every fan in the front row. Biding his time as guys kick Norman's ass, beat him up some more post-match, then leave. Glacier comes in the ring after the beating and claims that he rescued Norman. I can't remember if this angle had a conclusion though.
Another thing I remember but I don't know if it's a real memory. I feel like I remember watching a WWF home video that I rented as a kid, from the mid to late 80s where somewhere on the card was Bobby Heenan versus Gorilla Monsoon in some kind of debate or something. I remember space being cleared on the card for nothing but Bobby Heenan and Gorilla Monsoon taking to ringside, yammering at each other off mic for a minute or so until Bobby Heenan would throw his hands in the air and decide he couldn't take it anymore. This sounds too random to be a real memory until I remember that it's professional wrestling.
