Considering that they destroyed the Red Sox in Game 3 and had a chance to finish them in Game 4, the Yankees blowing a 3-0 series lead.
EDIT: And instead of Steve Bartman, the blunder should be "The Cubs blew a 3-1 in the NLCS vs the Marlins.

Babe Ruth. Losing some games isn't really a blunder to me unless you lose them in a really inept way, even if you lose 4 in a row to a hated rival.

Bartman.
Although it didn't necessarily cost the Cubs the game since they should have won anyway, I think Jon Gruden had less to do w/ Tampa Bay's Super Bowl win than people think. Dungy had already laid the groundwork there. Look what happened to him as time went on in Tampa.

Bartman (thought the Cubs blew that game anyway).
As for Grunden, I found it ironic that: a) he won the Superbowl vs his former team b) his final game as a coach was vs his former team (and that cost Tampa a playoffs spot)

I'm putting in a vote for Gruden, not because the Raiders traded him, but because they didn't bother to change any of their signals or audible keywords after he left, which meant he knew exactly what they were doing when they met in the Superbowl.
