Out of all the music videos I've ever seen, I can only remember two that were terrifying to me; Genesis's "Land of Confusion" and Mudvayne's "Dig".
The Genesis video I had seen when I was really young, and I only remember the scene at about 3:57, with the huge heads lining either side as the camera moves forward. I think the puppets are neat looking now, but at 6 or so years old? f*** those puppets, man.
The Mudvayne video I had seen at my Grandma's house when we were over there when VH1 used to air videos later at night (at 11 or so; the block might have been called 'Insomnia'). Anyway, the video came on and between the close up shots of the band in their makeup and the music that hits you like a ton of bricks straight up, it gave me nightmares that night. I don't like Mudvayne, but I've grown to like that song; scared the shit out of me, though.
So, FAN, what are some videos that used to, or still, scare you?

The music video for Seether's "Remedy" is pretty creepy. It used to really bug me, but I've gotten used to it, and think it's fun and over-the-top weird.
I love the song "One" by Metallica to death. The song's an absolute classic. But I can't watch the video, namely it's footage from "Johnny Got His Gun" without my brain squirming and me wanting to scream and thrash around in my chair until I hurt myself. I feel psychologically tortured every time I watch and see what is happening to the main character.

Those puppets in the Genesis video were no bother to me as they were in a UK show called Spitting Image. It's a classic of satirical comedy. I know it's an obvious answer but "Thriller" used to scare the shit out of me, particularly the scene when Jacko turns into a Zombie. I still get a chill at that bit now, not because it scares me any more but it reminds me of how much it used to bother me.

Also, the intro to Bohemian Rhapsody used to freak me out at the age of 4 or 5.

When I was young, there was something about the screaming baby at the beginning that freaked me out. The close-ups of Hodgson with the makeup also didn't help.

The Smooth Criminal long video, at least the musical portion, never bothered me much, but if you include the dramatic footage, Robo MJ was pretty scary for six-year-old me.

I'll echo the "Land of Confusion" fear. That was a weird one when I was a kid. Tool's videos for songs like "Sober" and "Prison Sex" were also creepy mindf***s when I was little.

Basement Jaxx's song "Where's your head at?" had one of the creepiest vids ever. Not too bad now though.
On the other hand, still can't watch the video for "Come to Daddy" by Aphex Twin. f***. That. Shit.

Dude, "Thriller" used to scare me, but it was not the zombie part that scared me, it was the intro when Michael turned into a werewolf. That scared me as a little kid that whenever that part came on, I had to leave the room. It doesn't anymore, though.

No music video that I know of scared me, but plenty have creeped me out. Like "Rock DJ" by Robbie Williams. Ugh, that video is horrible.
