Vader is one of the best answers people have said, simply because it is quite shocking he never had any belt in wwe whatsoever.
Some mentioned Crush, and where as his biker persona was never a champ to my knowledge, he did defend the tag titles as part of three man Demolition in the early 90s, although he never actually was part of the duo that won them. He was however pinned to drop the titles v hart foundation at summerslam 90.
So that may or may not count.

Actually now I'm thinking, surely none of those Oddities lads ever won a title? Joey Abs never won a title, I think, which is weird because he was the only one of the Posse who was a trained wrestler. Joey Abs won the Hardcore Title at Wrestlemania 16

Ha, with the exception of Vader, you really have to dig down the roster to find examples of non-title winners from that era. Look, a Scott Putski reference! To be fair, Scott Putski was totally the namesake for the Scott Putski Award which is as prestigious as any title WWF had to offer at the time.

Also, some of the ones people have suggested make me wonder in all seriousness if some people watched the show at all before 2001. Maybe it is just a memory thing. Did Hakushi make it into the Attitude Era? I don't recall him winning any titles, but I could very well be wrong. I also don't remember if Luna ever had the women's title. She did feud with Sable a lot and carry the hell out of her to no recognition, while freaking Debra and Kat had the title. I am actually kind of glad the Diva's title replaced the Women's belt in retrospect. Some godawful reigns.

I think Hakushi was gone by march of 96 after being branded by Bradshaw and Zeb, before the attitude era started up

Most of the early Attitude Era midcarders never won titles, as there weren't as many titles as there was later.
Obviously, if the 24/7 rule came in around 98, guys like the Boricuas or the Oddities would have traded the title.

I don't remember any of the UFC "guest" talent (Shamrock, Blackman and Severn) ever holding any championships. I don't think Malenko or Saturn did either. People already mentioned Shamrock and Blackman of the UFC group but a lesser known thing from that era is that Severn appeared on Raw with the NWA title and it was recognized on air and maybe even defended, my memories a little cloudy on that part. www.youtube.com/watch?v=KtsfuyW-28Q

I don't think the hardcore championship should count. I'm pretty sure *I* was WWF hardcore champion at one point and I was 10 at the time. So on paper a Godfather Ho had a better career than some of these guys. Even she was a hardcore champion.
