Technically, Orton beating him at TLC was clean, since Randy did it alone without help, and nothing in the rules says anything about handcuffing your opponent being illegal.

Roman Reigns pinned him clean with a spear in a Raw tag match a year or so ago.
I think Randy Orton has to be the person with the most clean wins over Cena. There were clean pinfalls at Wrestlemania and Backlash in 2008 and then Hell in a Cell in 2009 (plus several of the falls in their Bragging Rights match). The tables match they had in 2010 and the ladder match they had recently were also clean by the rules of those matches.
However, the Shell Shock is Cena's true Kryptonite. He took one from Ryback at Survivor Series 2012 and was then pinned clean (the Shield never touched him) even after a long delay and then at Payback last year Ryback beat him clean as a sheet with the move again in the first fall of their three falls match.

MITB 2010 against Sheamus, when Sheamus could escape the cage first only because the Nexus was keeping Cena from getting to the floor (and after Sheamus had tapped out while the ref was down).

All relatively clean as a sheet with no shenanigans involved.
Quite honestly though, as much a people complain about Cena going over, it's honestly rare for a main event face to actually lose clean without any sheningans on the heels part.

Been a while since I've watched it, but wasn't Punk in the STF at MITB 2011 in the first place because Vince and Laurinaitis had come out?
So he was the one originally distracted. Correct me if I'm wrong though.

I only consider standard rules, one fall matches to count as "clean" losses. There cannot be any outside interference, and neither can either wrestler use a weapon or otherwise break the rules in an overt manner.
So that means:

Cena is the champion going in, he comes in last, fresh, eats three finishers from three competitors and is immediately pinned. Cena looks like a beast taking that much to keep him down, but he also manages to lose clean and get over two heel wrestlers and a 170lber. Possibly my favourite spot in WWE history.
