Just making a guess but I can't think these guys using anything else but these names.
Doctor Death Steve Williams (I know he did the one appearance Mask deal but I don't think he was named.) Sabu? I don't recall him going by another name. Dean Malenko Kevin Sullivan (I know his nickname changed but they always called him Kevin Sullivan Junkyard Dog Ted Dibiase (Both Jr and Sr.)

Malenko wrestled as Dean Sarcoff for a while. JYD was Leroy Rochester in Tennessee and Big Daddy Ritter in Stampede. The DiBiases are a good call though.

Steve Williams was always Steve Williams I believe. Sabu was Samu in USWA. Dean Malenko posed as Ciclope, not sure if he had any other names. Kevin Sullivan's had more names than Mick Foley. JYD was Stagger Lee that I know of, probably more. Ted Dibiase Sr was always Ted Dibiase as far as I know. Jr might be a stretch since he was known both with and without the Jr, but yeah.

Do we really count Ciclope for Malenko? That was a one night thing to get on over on Jericho.
Some more I thought about.
AJ Styles The Brisco's Jake and Jerry I don't recall Sgt Slaughter being anything else unless we count Commissioner been wrong before. DDP always was DDP I think. Unless if we do could Ciclope for Makenko we would have to count DDP as LA Parka for one night. Tully Blanchard I don't recall Afa and Sika called anything else.

Someone else pointed out another name Malenko used anyway. AJ was Air Styles alongside Air Paris. Slaughter wore a mask as Super Destroyer 2. Dallas Page was always Dallas Page. I'm pretty sure Afa and Sika were only that, but with the way wrestling was it wouldn't surprise me if they were given different "savage sounding" names at different times. I wasn't sure about the Brisco Brothers & Blanchard so I looked them up. Evidently Jerry Brisco was Lord O'Beid, Jack was Uvalde Slim & Tully was the Midnight Stallion, but I'd never heard any of them before.

Sarge had a pretty boy gimmick similar to Jessie the Body Ventura and Superstar Billy Graham in the AWA before he donned the military gimmick.

I don't think you should count when a wrestler tries to disguise himself in order to get one over on his rival. Their disguise isn't really their wrestling character. It's the wrestler pretending to be someone else in order to trick or attack someone.

Apparently he wrestled under disguise as the King Killer at one point. I have no personal knowledge of this, though. I'd just be surprised if he wasn't involved in more than a couple disguise angles over time.
