From what I can tell, between Summerslam 1990 (vs. Dusty Rhodes) and his return at This Tuesday in Texas in December 1991 (vs. Jake Roberts) Savage had only one match on TV or PPV (vs. Ultimate Warrior at WrestleMania VII).
There are plenty of angles that explain away his absence (no-showing Royal Rumble '91 due to the Warrior angle; his 'retirement' for most of '91) but it seems a massive squandering of one of your top and most-over talents to book him in this manner. I mean, why no match at Survivor Series 1990?
Was Randy injured during this period? Any other theories (that don't involve Stephanie McMahon..?)

Just to clarify, Savage WAS maintaining a presense on PPV and TV, just not in the ring. He also was working a lot of house shows against Ultimate Warrior throughout autumn 1990 right up to WrestleMania VII. I wonder why he didn't captain a team against Warrior at Survivor Series?

This is during the period him and Liz actually broke up isn't it? Nah, that was more like midway through '92. Early '91 was around the time Randy apparently wanted a bit of time off the road to come off steroids so that he and Liz would have a better chance to start a family.

I know him and Vince had a big blow-up after The Main Event match against Hogan. Randy even threatened to jump to WCW.

Saturday Night's Main Event #28 - 10/13/90 - Randy Savage (w/ Sensational Sherri) defeated Dusty Rhodes via count-out at around 9:30 when Savage came off the top to the floor with a double axe handle after Dusty became distracted by Ted Dibiase and Virgil at ringside attacking his son, Dustin, and busting him open; during the bout, Dibiase and Virgil appeared in the crowd and paid off the entire front row; moments later, Dibiase tried to pay Dustin off but he ripped up the money and threw it in Virgil's face; Dibiase and Virgil then took a seat beside Dustin; after the bout, Dusty covered his bloodied son and was stomped by Dibiase and Virgil
Wrestling Challenge - 12/2/90 - Randy Savage (w/ Sensational Sherri) pinned Dale Wolfe at 1:53 with the flying elbowsmash
WWF Superstars - 12/22/90 - Randy Savage pinned Koko B. Ware at 2:44 with the flying elbowsmash
He was regularly working house shows at this time as well. He did however get injured leading up to Wrestlemania VII and missed several weeks.

After Sid Justice was injured early in his WWE run, Savage replaced him on several house shows against Jake Roberts. According to reports, he wrestled one match under a mask as Mr. Madness. They wrestled for much of October and November on the road.
A few days after WrestleMania 7, Randy Savage had a match with Rick Martel which made it onto one of the Coliseum Videos. Sean Mooney mentioned how WWE officials allowed Randy to wrestle the match because the fans had requested the match. Savage had not been reinstated on TV yet.

I believe it was over the way Savage was being booked, losing to Hogan and then being put in a feud with Rhodes in-which he was going to lose at Mania.

Randy was such a workhorse after his arrival in the WWF, that I wonder if after 6 years or so Vince just wanted to give his body a bit of a rest
I think Savage and Liz separated in 1992...so 1990-1991 could've been a period of marital discord

This site doesn't list any Duggan/Madness v. Roberts/Undertaker matches. However, there are several Duggan/Savage v. Undertaker/Roberts matches that took place in December, which was after the reinstatement of Savage. Maybe they originally were advertising Mr. Madness because Savage had not yet been reinstated, but they went ahead and switched it to Savage once he was reinstated.

I actually remember him saying that they allowed the match to happen because "the contract had already been signed prior to his retirement match." as a child it seemed odd to me since that meant that both were heels when they signed the contract for the match and heels never faced each other.
