Scott Steiner being all over WWE TV from November - March 2003, main events the Rumble and No Way Out and isn't even remotely close to a Mania match. That is, unless they expected that Nowinski feud to light the world on fire.
Come to think of it, considering all the TV time and promotional work Test and Stacy were doing around that time, it's odd they were gone from Mania 19 as well.

Mr. Perfect not being on Wrestlemania X8 despite a excellent showing in the final four of the Royal Rumble.
Could of faced Kurt Angle or Steve Austin.

Also, to add to the thread: Booker T and Big Show missed SummerSlam 2005. It got worse in 2006 after ECW started up. The Undertaker, of all people, missed SummerSlam. Enough said. Oh yeah I remember that. Wasn't Undertaker vs. Great Khali in a Last Man Standing match booked for Summerslam then it was put on the pre-PPV Smackdown. Big Show not at Wrestlemania X8 (and lifting people in WWF New York) was quite noticeable at the time. Also, I seem to remember Undertaker and Kane scheduled to challenge Matt Hardy and MVP for the WWE Tag Team title. Looked like it was gonna be at No Mercy but was on an episode of Smackdown. I can see why it wasn't on the PPV (having Undertaker on a PPV title match and not get the title) but I think it could have been quite a unique match for the show. And I thought it was odd that Christian was left off Summerslam 2003. He had recently changed his look, was the Intercontinental Champion and was prominently featured on Raw at the time. He just came off a feud with Booker T that was cut short I think due to Booker's injury. From what I remember the card was stacked that year too so no time for Christian. The Matt Hardy vs. Zack Gowen match was cut (not sure if due to time or storyline after Lesnar destroyed him on Smackdown) to a forfeit win on Heat. Hardy's MF'er Shannon Moore would "main event" Heat though against Rey Mysterio for the Cruiserweight title.

HHH at Vengeance 2001, he was on the the poster and they also used him for the commercials and yet no appearance. That felt strange. The theme of the PPV graphics was sledgehammers smashing into metal too. If he returned at Vengeance, would he have been in the championship tournament or in a squash return match??

Kinda weird, we live in a world with so many former World Champions, it's odd when any of them are off the card. For example, Orton not even being booked for Summerslam 2013 before he cashed in was weird.

Apologies if it's been mentioned, and it might not fit the topic, but leaving Chief Morley+Lance Storm vs Kane+RVD (which featured a double-swerve-turn by the Dudleys) off Wrestlemania 19 was a bad decision, although that card was absolutely stacked from top to bottom so they can blame it on time restraints.
Daniel Bryan vs Sheamus from WM 28 is another obvious 'shifted to the pre-show' mistake.

Stone Cold would have told them where to stick a match between him and an over-the-hill WCW midcarder, he had enough problems with facing Scott Hall who was a top WCW guy just coming out of his prime. Not sure Mr Perfect making a token 'surprise run' warranted a Wrestlemania payday, his conduct backstage justified this.

WWE must have expected his match (with Kofi Kingston vs. Legacy for the World Tag Team Championship) to be voted in as the "tag team match of choice", though it ended up being Cryme Tyme vs. Miz & Morrison. Then Punk and Kingston won the title the next night anyway.

No Jericho or CM Punk at WM XXX seemed weird. Not really. Jericho has started to be a guy who just sort of shows up from time to time when he feels like it or the 'E really needs him. They were calling a ton of audibles at the time, yes, but I can't really think of anywhere Jericho would have fit that wouldn't have felt forced. As for Punk, I've said numerous times that I hate Punk isn't on TV regularly anymore, but there wasn't anywhere on the card for him and he was treading water at best at the time, so I take solace that I don't have to see Punk being dragged down with all the other nonsense going on right now and can instead fondly remember 2011 era Punk.

Triple H not having a match at King of the Ring 99. He was in Chyna's corner for her tournament match, and he cost Rock the title match against Undertaker, but had no match of his own on the card. That was due to an angle during the show - Vince announced him as his partner in the main event when it looked like Shane couldn't make it, but then Michaels threw him out for getting involved in the title match, then it was discovered Shane was faking his injury.

Undertaker at Survivor Series 97 is the obvious answer. The Undertaker at Survivor Series 97, guess he was scared of kane lol. Undertaker was the special guest enforcer for Bret Hart & Davey Boy Smith vs. Vince & Shane McMahon after the show.
