Yep, people will call her a trendsetter for setting the stage for the various pop stars that tease bisexuality to sell records rather than talent. At least MJ had some decent songs. She's just as talented as MJ when you cut away all of that. Don't downplay her importance because you may not like her or her music. Yup. I'm not the biggest Madonna fan, but it's hard to imagine the 80's or 90's without her. Her and MJ made MTV.

For global impact... Queen Elizabeth II is really high up there. Runners up would either be Pope Francis (God, please, not for a while) or Bill Clinton. Regardless of what you may think of Clinton personally, I don't know if there is a living US President with a higher global profile than he.
For celebrities... if we are looking at shock value and impact, it would have to be someone like a Jennifer Lawrence. For respect over time, Chris Lee, Sean Connery or Ian McKellan would head the list.
