For f***'s sake 1989 WCW/NWA. Watching Steamboat Flair two out of three falls on the network and saw this (they spelled Flair right it was flickering and I wasn't going to spend 2 hours trying to get the whole word right. ) I'm sure this wasn't the only time by his actual employer. Any other notable examples?

hmmm. Could it be for a brief period of time Flair may have actually tried to be "Rick"
My dad for instance...his name is Randy but everybody always calls him Rand. Even on invitations and such. One month he made a concerted effort to try to get everyone to call him Randy but it was no use. Still Rand.

Not a Flair example, but magazines used to spell Luger with an a, as in "Lex Lugar" and Bret Hart was spelled "Brett Hart", but I do believe I have seen those on tv as well, so for some reason, a spelling change was made

Dunno how that happens, but I remember the AWA shows from the late 80's always misspelling guys' names. They spelled "Teijo Khan" more ways than I can count. My favorite was when they just got lazy and spelled it "T-Joe Khan".

Rotundo/Rotunda and Hennig/Henning are ones I see a lot, too, even in official materials.

When the Rockers were first starting out, Shawn Michaels was spelled Sean Michaels by many magazines . I think once they found out that there was a porn star named Sean Michaels they went to the Shawn spelling.

Well, because he was called "Rotundo" in WWE and "Rotunda" everywhere else, so that one's not really an error. The Hennig/Henning thing is definitely because for some reason, everyone liked to call him "Henning".

"Rey Mysteric" made his debut at Starrcade 90 and it was funny hearing J.R. refer to him as such.
