I'm just thinking about this since the WWE is gonna stand pat for a few months with the title situation, and how I don't think they are fully invested in Seth Rollins, even though the heel figures on the show keep talking about how he's the future.
So... we already have had two MITB losers in the history of the concept, so what about the first-ever "never cashes it in". Frankly, the setup for this would be more interesting than just having Rollins try to cash it in on Cena or Roman Reigns, only to get swatted down, which would immensely hurt his credibility, in spite of the fact that his character is that of the whiny jerk who only does things when it benefits him the most.
I was thinking that Rollins doesn't cash it in on Brock Lesnar for storyline reasons, and then after WrestleMania, he tries for either possible champion afterwords (the company's preferred choice in Roman Reigns or the fan's preffered choice in Daniel Bryan), only to have some complications result in him not cashing it in on time.
But, in spite of all this, he's still way ahead of the whole "not being booked like a complete idiot" philosophy which has all but doomed Dean Ambrose.

I see him cashing in... but failing, as some sort of karmic poetic justice for his actions since betraying the Shield.

To me, this is the most likely outcome for Rollins in his MITB cash-in... he gets the opportunity, uses it, and then loses. Personally, of the three Shield members, I view Rollins as the least-likely to ever be WWE Champion.

I would love to see Ambrose(or roman) win the belt and beat Rollins with a flash Rollup as the ultimate karma for turning his back on his friends

In kayfabe, he should be complaining about Brock not being around much, since he has fewer opportunities to cash in than every other MITB winner.
It doesn't really matter if he loses the cash-in, he's been booked stronger than anybody, which is usually the opposite of what they do with MITB holders

This idea works better with a face MITB holder I think. The MITB deadline is coming up, and either the champion or another heel blindsides the face and puts him on the injured list causing him to miss the deadline and the MITB contract expires.

Just have Brock push Seth against a wall and Heyman gives some speech about how he should cash in now and take your beating now when Brock is in a "good mood" than angering Brock by inconveniencing him with a surprise title match and receiving an even worse beating.

Would be a neat way to implement the sort-of partnership implied, that if Rollins wants to be a Heyman Guy or whatever, he ought to take a beating from Lesnar like a man.

Personally, of the three Shield members, I view Rollins as the least-likely to ever be WWE Champion. I completely disagree. Out of the three members of the Shield, he in the post-break up has been knocking it out of the part the most. Sure, in the long run they could essentially f*** it all up, but as it stands RIGHT NOW, he's in a good position to be put in that spot.
