My plan for a different outcome to the 2015 rumble would be
Have Sheamus return, but he's not an official entrant. Instead he comes down towards the end of the match, cowardly eliminates the last two wrestlers in the ring (Bryan & Reigns) and celebrates like he has actually won the Rumble.
This would
1) Establish Sheamus as a top heel 2) Allow Reigns to win without having to be the bad guy by eliminating Bryan to win 3) Gives WWE the Bryan v Sheamus return match at Wrestlemania that they are desperate for.

This is an ending to the Royal Rumble that I've always wanted to see. Yeah it would be a shitty ending that fans would hate but, damn it would to cool.
There are two people fighting in the ring before number 30 enters. With just 10 seconds left on when the two guys have eliminated each other. And then number 30 comes and has just won the rumble without having been in it.
Of course, it would be a heel would be booed like never before as he gets on the mike talks about how he outlasted 39 other guys to win.
Yeah it would never happen but, it would be fun to see.

One guy isn't eliminated but no one realizes it. Someone else then wins the Rumble. Next night on Raw they're like "Hey I wasn't eliminated" and the Rumble technically lasts like 23 hours long.
You could do multiple variations on this like having the guy sneak up and eliminate the fake winner, having him announce it, or even better having a game where the two guys are like "oh shit I might get eliminated" and try to avoid rings for the rest of their careers.

Trying to think of some finish where either the ring itself collapses (which would have been cooler if big show hadn't done it first) or do the old school territory finish of the ropes snapping some poor soul taking a plunge to the outside as a result.
Or, while I'm being crazy, have a female enter (which a few of them have already I believe) win and then the main event of Mania is a Divas title match.
I'll show myself out.

My "most wanted" ideas have already been mentioned, but what about a tag team (hell, maybe even an entire faction) that lasts till the end and they can't decide which of them gets eliminated?
As long as it's not treated as a comedy spot, it could be kind of interesting.

Maybe have Ambrose enter early and be in a bad way against a group of authority stooges perhaps including Rollins until Reigns enters around 23. They clear house until they're the final two, after a moment of "what now?" they look at Rollins on the outside and both exit the ring at the same time while Rollins sells it as the worst thing to ever happen to anybody ever. Shield triple-threat at WM.

For me: The random lower card "jobber" finally proves ANYONE can win the Rumble. Shoulda done this with Santino. Have him win the Rumble, and over the next few weeks, show vingettes of him training hard, have him do a sit down interview with Cole. No more comedy from Santino, don't get me wrong, he's still a face, but he's dead serious and realizes *this* is his one shot. He goes to Mania, puts up the fight of his career, taking the surprised/offguard champion to the limit, before finally losing. It makes Santino a million times more credible, and more importantly creates the illusion that anyone really can win the Royal Rumble. I fail to see how that wouldn't help future 'Rumble events look like a MUCH bigger deal.

People always bring up the #30 winning without doing anything and to me it sounds like one of the dumbest ideas ever. But multiple people bring it up every year... #1 and #2 eliminate every single other person in the match and are the final two. That's kinda what happened in 1999... well I suppose they didn't eliminate everyone else themselves but still.

Final two are a plucky underdog face and an evil top heel. The plucky underdog face turns heel by eliminating himself and revealing himself as a lacky, and he'd been subtly protecting the top heel from elimination the whole time.

In my fantasy booking that takes place in my head...
Prior to the Rumble, Paul Heyman says that the authority have been throwing curveballs at Brock Lesnar during his whole reign (in my fantasy booking they have been. not outwardly, but always giving Cena more shots, the triple threat at RR etc) and they say that at the Royal Rumble Brock is going to make history and at the same time make a statement. Everyone assumes it's about his title match.
Brock retains his championship in a match earlier in the card.
During the actual Rumble match let's say after #23 has entered, there are about 8 or 9 people in the ring. Earlier in the night random wrestler was shown beaten down, and needs medical attention. That person had the 24th slot (unbeknownst to casual viewers).
#24 hits. It's Brock Lesnar.
Heyman gets on the mic and says that Brock is tired of being mistreated and to prove his dominance he will be the first WWE Champion to win the Royal Rumble ensuring there is no number one contender. Pick who the last people are...Bryan, Reigns, Ziggler, etc....they all try and work together to eliminate Brock, but he's in beast mode, it comes down to him and whoever you want and he sends them over the top rope. Brock Lesnar wins the Royal Rumble becoming the first champion to do so.
The next weeks on Raw are chaotic as The Authority scrambles to make things right. They say they can't take away Lesnar's win, but they will ensure he has a challenger for WM. Heyman is adding to Lesnar's conquering speech with the Undefeated Streak being broken, Cena being dominated, Retaining his title at the Rumble, and making history in the RR match itself. Adding to his accolades and giving an ominous warning that he's not done conquering yet.
I devised this in my head when the Elimination Chamber was still the next PPV, and The Authority set up the match to decide the #1 contender. They pick the last 6 people who were in the Rumble (just to make it simplified for fantasy purposes).
So we get Ziggler, Ambrose, Rollins, Reigns, Bryan, and Wyatt in the Chamber.
In my head it comes down to Ziggler and Bryan as the last two. They start going at it, the announcers are building it up as "one of these men are going to Wrestlemania!"
Lesnar's music hits.
Heyman comes down with bolt cutters, snaps the chain off the door to the chamber and Brock Lesnar enters the EC and destroys both men.
There is no clear winner as it's a no-contest.
The Authority then make the WM Main Event a triple threat match due to Brock's interference and it's: Lesnar vs. Bryan vs. Ziggler
This makes Brock look like an absolute beast of a champion, unstoppable and bent on destroying everyone. This makes the winner of the match at Wrestlemania look that much stronger for overcoming the Beast Incarnate.
Replace the last 2 EC people with whomever you wish as the triple threat competitors.

The first 29 men somehow all take each other out before #30 is even called. So, all #30 has to do is walk into the ring, and he (or she) is going to WrestleMania. I present to you, something I've referenced every year since 2012 when that rumoured Jericho finish first came out...

The person that does the "cool elimination save" spot needs to actually win the match. I assume you're talking about the dangerous spots that Kofi does every Rumble, right? That's way too risky and only done by midcarders who are sure to not win it. One slip and the whole plan is done. The spots that HBK, Misterio and Jericho usually do where it looks like they're done are much safer than the Kofi spots.

A top, top babyface is tipped to win the rumble or heavily made to be an underdog that you just know is going the distance, Rey in 2006, HHH in 2002 or Bryan/Reigns this year....only to do a Santino/Warlord and get tossed out in an embarrassingly short time.
At first the wrestler is destroyed mentally but his friends (Cena ect) try to pick him up, he gets sympathy from the fans, picks up the odd win and his consolation prize is an IC shot at mania, he does well but narrowly loses and snaps going full on Backland style insane heel, maybe even interfering in the main event against the guys who took what should have been his rightful spot if it wasn't for that one mistake.
Killing joke style storyline, all it takes is one bad day to change someone forever.
