Not like, in a rage and call them crazy or retarded or something. But legitimately having a conversation, explaining that there's things you notice that leads you to believe there may be a mental condition going on.
I have a cousin who constantly remembers things wrong, creating things that aren't here. For a silly wrestling example that we'd all understand, he talks about how he used to watch WCW, but got bored because Hogan was tapping out to Flair in the Figure 4 every week, and he's dead serious.
We just had a conversation where he claimed that, 2 and a half years ago, I told him I weighed less than him and had him try on my pants to prove it...I also remember this conversation, and I told him I had gained weight, and he bugged me into letting him try on my pants. He generally twists things into this myth-making, self-assuring narrative, where he's always in the right, or being wronged somehow. It's like that episode of South Park where Jimmy writes the fish dicks joke and Cartman keeps telling the story where he wrote it and did something awesome and it keeps getting more and more outlandish. It's not that extreme, but still, definitely reminds me of it.
I used to just get irritated and think he was being a prick, but at this point, I'm concerned. I know he's had some rough experiences that could certainly contribute to mental illness. Have any of you ever had a really tough conversation like that with someone? I'm not sure I could even begin to try, because this guy is so convinced that his thoughts are true.

Yes, my gf is definitely mental ill, yet I've endured(she's finally seeing somebody about it) and I made the mistake of saying something she said was "crazy" last week, actually....oof, big mistake, it's been brought up like 20 times since then.

Yes, but when the guy that lives in the mirror that looks just like me denied it, I believed him.

I work in a mental hospital, so yes I've had to break that news to people once or twice.

I've been involved with signing papers twice for two different people that required against-their-will-at-the-time help.
Both have thanked me multiple times over the years.

I tell that to myself. I don't think I'm bad enough to be medically diagnosed with any mental illness, but I'm pretty sure there's some level of chemical imbalance up there.

Just to be clear, there are varying degrees of mental illness. Technically, depression is, but I wouldn't give it the same stigma as being a bigot (which some experts believe is also a mental illness) or having serious personality disorders (like narcissism). It's okay to point out that someone needs/can benefit from help, but it's a delicate procedure. My aunt had a stroke last year, and it took us a that whole year for us to finally convince her that taking antidepressants for her mood swings was needed and not "failing". She can be left alone know with fear on her family's part that she's not gonna hurt herself or wreck stuff. My ex-boyfriend, on the other hand, is thirty years old and likes to flirt with/sleep with high schoolers. I told him he was mentally ill too, but for a completely different reason, and much different tone in my voice.

I've never been the one to bring it up, but I've had a few ex girlfriends ask me for my opinion on their mental health status. I'm not a psychologist nor am I any type of medical professional, but I really tend to get treated like one by the women I date.
I convinced one of my ex's to get help and even took her to appointments. Turns out, she has PTSD stemming from her childhood and is also bipolar. She still calls me sometimes with thoughts of suicide and stuff like that. It's a really frustrating situation in my life because she's always extremely manic or depressed and ALWAYS tries to contact me with invitations to her house or to talk about her problems. If I avoid her, she goes into a depressed episode again. It's gotten to the point where it's adversely affecting my own mental health. Actually, if someone has some advice on how to deal with it, that would be super.
My most recent ex just randomly asked me one day, "do you think that I'm Bi-polar?" I basically told her "I don't see it, but since you're asking...maybe?" After a few more weeks together, I told her that I do see Bi-polar tendencies in her.
I think there's something about me that is alluring to people like that. I've yet to have a relationship with somebody that was mentally stable.
