I was just reminded of how I began to dread Hernandez matches for fear of him paralyzing an opponent with yet another sloppy Border Toss. Hernandez is probably the most prominent current-era wrestler I can think of with a reputation for being unsafe in the ring, but there's definitely more. Basically, anybody you would refuse to wrestle based purely off of their reputation as a worker.
Low Ki, I dunno how he is nowadays, but I recall him having a reputation of getting a little carried away with stiffness-for-the-sake-of-stiffness (a problem endemic to a lot of indies, to be honest), particularly with his kicks.

Ahmed Johnson. And....man, this hurts, because I like the guy, but when he feels like it, or when someone just pisses him off, Jonathan Gresham can drift straight into "pointlessly stiff prick" territory.

I can see New Jack but why Goldberg. Ask Bret Hart. Eh, Bret was just coming off a concussion at the time and he continued to wrestle for a while after Goldberg injured him. Might chalk it up to a lack of concussion awareness (specifically how dangerous they are) though.

He also had matches with Terry Funk after the kick, Bret admits on his book that it was reckless, he should've taken a year off in order to rest and heal, it's too bad that back then we didn't had the concussion protocols.
I'm gonna pick Billy Robinson because he would shoot on his opponents and he also knew hooking and could f*** people up. Wrestling is about trusting each other. Superstar Billy Graham once told a good story about taping a blade on his fingertips and threatening to cut Robinson if he even dared to go against the script.

I never saw Goldberg as consistently unsafe. Just stiff. And the Bret Hart kick was a total accident. I never look at that kick and say "Damn that unsafe brute!"
Just an accident that happens in wrestling all the time. A punch or a kick connecting a little too closely or timing just being a bit off. Sucks, but it happens. Just saw Goldberg as more rough, than unsafe.
Now the easy answer is New Jack. It's always New Jack. The man has admitted to wanting to injure, and kill opponents because either he didn't like them or he was in a bad mood.

Bob Holly blasted dudes, but I am not sure if that would be considered unsafe so much as just rough, since I think it was more painful than dangerous.
Koji Kitao is infamously stiff and not a very giving opponent unless you stiff him right back until he knocks it off.
Umaga used to be very undisciplined and often hurt people, like in the HLA segment.

Vader worked a stiff style as his first monster push was in Japan. I've heard he felt terrible when he legitimately hurt people. If I recall, Steamboat once commented that he thought Vader didn't realize his own strength sometimes. When Vader injured WCW enhancement wrestler Joe Thurman, he was broken-up about it. If you watch it back, Vader shows a lot of concern for him. Supposedly Vader broke down and cried afterward because he was so upset about it. So in this way, Vader was never malicious and wanted to protect his opponents. At least you can give him that.
