From the top of my head... John Cena's first Title victory in the WWE when he beat the Big Show for the US Belt. WM 20 also Triple H and Shawn Michaels wrestle and the World Heavyweight Title disappears for a few months from the same show Alicia Fox wins the Divas belt (Fatal Fourway) Rey Mysterio wins the World Heavyweight Tital (fatal fourway) Sheamus wins the WWE Title (fatal fourway)

Too many to remember. Here are some that I do remember...
NWA World Title Tournament - Chris Candido wins the NWA World title. ECW Gangstas Paradise - The Pit Bulls win the ECW Tag titles. ECW The Doctor Is In - The Gangstas win the ECW Tag titles. WWF Summer Slam 97 - Steve Austin wins the WWF IC title, Bret Hart wins the WWF World title. ECW - 10/18/97 - The FBI win the ECW Tag Titles. MLW Genesis - Shane Douglas wins the MLW World title. ROH Crowning A Champion - Low Ki wins the ROH title. MLW Reload - Satoshi Kojima wins the MLW World title. CZW Cage of Death 7 - Super Dragon wins the CZW World title. ECW One Night Stand 06 - Rob Van Dam wins the ECW World and WWE World titles.

Aside from Windy City Pro Wrestling's "Battle of the Belts XIII" (where almost all of their titles switched hands), I've seen one major title change live in person.
The Undertaker beats Sycho Sid for the WWF World title at WM13.
The year before (November 1, 1996), I witnessed my first live "Dusty finish" at a WCW show. Harlem Heat beat Hall & Nash for the WCW World Tag titles...not really. Nash had brass knuckles, Booker flattened him with the axe kick and made the cover. Nash planted the 'knucks on Booker while he pinned him. Fans cheered when it was announced the Outsiders retain by reverse DQ. (Hammond is nWo...for life.) Booker called us a bunch of racist [so-and-sos].

I was there when Eric Bischoff took the World Heavyweight title out of a briefcase and awarded it to Triple H.
And I saw Dean Malenko beat Ulitimo Dragon for the Cruiserweight Title at Clash of Champions.
I think that's it. I usually go to house shows. Cheaper Tickets, smaller crowds and more wrestling.

-Seth Rollins wins WWE Title cashing in Money in the Bank in Reigns vs. Lesnar match.
-Daniel Bryan wins Intercontinental Title in 7-man Ladder Match.
-John Cena wins US Title from Rusev
Wrestling for Charity:
-Vennis DeMarco wins WFC Title in a 4-way elimination match.
-The Sotello Syndicate (Omega and Pinkerton) defeats Kryptonite and Jack Brinx (sub for The Hurricane) for the WFC Tag Titles.
Phoenix Pro Wrestling:
-Bobby Hart becomes the inaugural PPW Champion, defeating Jeff Cobb in a tournament final.
-Hart vacates the PPW Championship; Drake Frost wins the title in a 4-way match.
-On Friday, is the tournament for the inaugural PPW Tag Titles, so I'll get to see that.
Pro Wrestling Revolution:
-El Hijo de Trump defeats Blue Demon Jr. For the PWR Title.
-Los Luchas win the PWR Tag Titles.

I saw Miz and Morrison defeat Matt Hardy and MVP for the tag titles and Kane win the World Heavyweight championship at MITB 2010, those are the ones I remember but there might have been another one I'm forgetting.

Oddly, the only one I can think of offhand was RVD taking the hardcore title from Tommy Dreamer. I did see a Dusty Finish between the Headshrinkers and Diesel & HBK though.

I almost saw Triple H & Shawn Michaels win the Tag Title from the LOD at a house show in October of 1997, but the LOD cheated so the referee reversed the decision and gave DX the win by DQ.

I was at a Nitro to see Ultimo Dragon win the cruiserweight title from Eddie Guerrero.
We were at GAB to witness JBL win the WWE championship from Eddie in a bullrope match. (We were pissed)
I was also in attendance when Ron Simmons beat Vader for the WCW title. (I just stood there with a shocked open mouthed look on my face while the place went batshit crazy)

Cena beats Orton in the no DQ Iron Man match at Bragging Rights.
Yep, I had to watch those two for an HOUR!
That's endurance. Everybody in that arena should have gotten a replica title for putting up with that.

Only 2 I can think of are Kidman/London beating the Dudley's for the WWE Tag Team Championships on SD and Alundra Blayze beating Bertha Faye for the Women's belt at a Raw taping in 1995
