If a QB is tackled behind the line of scrimmage on a designed run play does that count as a sack?

If he established himself as a runner, it goes as a tackle. If there is any doubt to whether he was to run or pass it is a sack.

If he has the ball and is the one running, it doesn't count as a sack. Just a tackle for a loss of yards. On the other hand, you can have a sack on someone else besides the quarterback if they're the one passing (like a wide receiver or runningback in a wildcat formation) and it's at or behind the line of scrimmage.
Sacks can also be recorded if a defender forces the passer to fumble behind the line, even without a formal tackle, or run out of bounds behind the line without throwing the ball way.
