"Roseanne" named after Roseanne Barr/Arnold but in show last name was Harris/Conner
"Reba" named after Reba McEntire but in show last name was Hart
"According to Jim" named after Jim Belushi but in show last name was Orenthal
"That's So Raven" named after Raven-Symoné but in show last name was Baxter
"Ellen" named after Ellen DeGeneres but in show last name was Morgan

The Wayans Bros. Shawn & Marlon's last name on the show was Williams. Similarly, on the Jamie Foxx Show, Jamie's character is named Jamie King.

But there was another Betty White Show in 1977, where Betty plays someone not named Betty White. AND it was another MTM Production! (which has done this already to Bob Newhart and Mary Tyler Moore) How about the last season of Too Close For Comfort being called "The Ted Knight Show"? Never mind he was still Henry Rush on the show. Regarding an earlier selection, and somewhat related: come to think of it, how often on Fat Albert did they refer to Bill (and Russell's) last name anyway? And since the rest of the gang was not related, they couldn't be the kind of Kids the title called them, could they? Would The Carol Burnett Show partially count as Carol doesn't play herself in most skits? ;-)

Here's the opposite situation...Young Rock. On the show he is Dwayne Johnson, his real name instead of the character he played which is the title.
I'd be curious if that has ever happened besides here.

How about the last season of Too Close For Comfort being called "The Ted Knight Show"? Never mind he was still Henry Rush on the show. This is weird because there was briefly another The Ted Knight Show in 1978 completely unrelated to Too Close For Comfort. On that show he was named Roger Dennis. The Colbert Report is in the Seinfeld zone (or maybe the Young Rock zone) since he was playing a character of Stephen Colbert, pundit, and not his real personality.
