Henry Winkler as Barry Zuckercorn on Arrested Development did a couple of callbacks to his Fonzie character, including I LOVE this show, I started watching reruns around 2007, and I never got that until now.

One of the most classic episodes of Batman: The Animated Series (Beware The Grey Ghost) had Adam West appear as a washed up old actor whom was best known for starring in a campy old live action super hero TV show.

In the original 13 Ghosts movie from 1960, there's a creepy old housekeeper who everyone thinks is a witch. She's played by Margaret Hamilton, who played the Wicked Witch in The Wizard of Oz. {Spoiler}{SPOILER: CLICK TO SHOW}It turns out she actually is a witch, but a good one this time.

I’m not sure which is my favorite of these between…
Neil Patrick Harris playing Neil Patrick Harris as a drug addled sexual deviant in Harold & Kumar Go To White Castle
Or
Michael Cera playing Michael Cera as a drug addled sexual deviant in This Is The End.

Would Richard Belzer appearing as "Munch" on 11 different shows count?

Does it count if it's just a throwaway joke? I'll never forget John Lithgow and William Shatner on 3rd Rock From the Sun commiserating about the horrifying experience they each had when they were on a plane and looked out the window and saw a gremlin on the wing.

Boris Karloff as Byron Orlok, retiring elderly horror icon who feels left behind and out of date in the current real-life brutality of the world, in Targets (1968).
Not quite Karloff's final film, but almost, and a really good tribute to the great man and his career.
Here's a little clip, as he relates a spooky tale.
