If this happens, I realistically see it as more of a heel turn for the Rock. Rock says he did it because Roman was "getting too big for his britches" as Head of the Table. But it really comes down to Rock feeling insecure/threatened by the possibility of Roman's legacy overtaking his own. But... Rock's already a heel?? You're right, I meant a default face turn for Roman. Kind of like when Triple H (heel) turned on Seth Rollins (heel) to help Kevin Owens. Rollins then became face

Roman trades the belt to The Rock for a really good cheese pizza. The Rock eats the belt, and lays down on a table. The Rock then turns into the guy from the board game Operation. Cody removes the belt from The Rock’s stomach using a giant pair of tweezers. He manages to do so without bumping the sides with the tweezers, and setting off the buzzer/making The Rock’s nose light up. Triple H comes out, and loudly chews gum for 3 minutes before declaring Cody the new champion. Cody celebrates by throwing bottles of motor oil to fans in the crowd.

The absolute worst would be him being "awarded" the title due to "forfeit/someone stripped of the title " (ex HHH 2002, Orton 2007).
Here are a couple of other bad examples.
Winning his FIRST WWE championship on a house show (if they even still do those) or other non-PLE event.
Or
He beats anyone NOT named Roman Reigns at this point.

The match is falls count anywhere in Philadelphia. They quickly brawl out of the arena, fight through the streets until they get to the liberty bell, and Cody rams Roman into the bell and pins him.

Roman hands over the title to Cody before the match saying, "I acknowledge you...booker man's...son."

There had better be a spot where they brawl into the old ECW Arena and Heyman tries to hit Cody with one of his big brick 80s cellphones.

there should be a ring setup there for no reason... and that's where Cody pins Roman.

While Cody gets on the mic and as he is ramming Roman into the bell, he sings: “Ring my bell.”

Roman and the rest of the Bloodline run out of gas on the way to WM, and Cody has to wrestle Sika for the belt instead.

Cody is about to win the title. Cody has Roman pinned, but then AVA RAINE comes out holding a baby in her arms! She says that Cody got her pregnant but is refusing to pay any child support! Cody gets on the mic and says that there's no way that baby isn't his, because "my pull-out game is impeccable". Rock grabs the mic from Cody and says he will defend his daughter's honor and challenges Cody to an impromptu 'Child Support on a Pole' match, bro! Roman is so confused that he slinks out of the arena with the title.

On the kickoff show, Triple H strips Roman of the title for not defending it in the previous 30 days. He declares the title will now be defended under 24/7 rules, and hands the vacant title to R-Truth, who is immediately chased around the arena by the usual gang of idiots. The title changes hands multiple times via typical 24/7 shenanigans. During R-Truth’s 17th championship run, he challenges Cody to a game of WWE 2K24, where the real championship is in the line. Cody beats R-Truth because his R-Truth couldn’t figure out how to plug in his old school SNES controller. Before the ring announcer can finish saying, “Here is your winner and new…”, Priest cashes in Money In The Bank and wins. Priest immediately sells the title to Ted Dibiase.

But somehow thanks to R-Truth, the wires got crossed and Priest sold the title to Ted DiBiase Jr.
