There's a Cage of Death (about number 5 I think) where Jack Evans gets eliminated by being fully flipped backwards over the edge of the Cage, and not quite managing to grab on to steady himself and landing flat on his back on the floor.
No, I don't know how he's still wrestling 20 years later either.

Every Mike Awesome v. Masato Tanaka match was basically a slide show of wince inducing spots.
Doesn't seem to get talked about much now, given how things turned out for Awesome.

Two for Vince McMahon immediately come to mind in the "was that really necessary?" category:
1) At St. Valentine's Day Massacre 1999, he takes a hideous-looking bump from the steel cage to the announce table, landing spine-first directly on the edge of the table, causing his head and neck to completely snap back. In one fell swoop, he probably suffered a horrible back injury, possible internal injuries, a concussion, and whiplash. And that was within the first few minutes of the match.
2) At WrestleMania 22, Shawn Michaels uses a ladder to hit Vince in the head. But Vince doesn't even try to put his hands up, he just takes the top of the ladder square to the forehead.

I have always maintained that when putting the match together, the two would flip a coin on who had to take the powerbomb directly onto the back of their necks. Those two were absolutely disgusting to each other.

He was scared of the cage bump but trusted rikishi and said he barely felt it as the big guy positioned it so well. *In this interview he was anti Martha Hart as "people do die" so you know, even then he was quite crazy.
