Moonsaults, SSP's are tough to hit just right and pretty often people on the recieving end take a pretty good shot from some part of the body.

A buddy of mine pissed me off once, so I threw him on the ground and gave him a Steiner Recliner. It didn't take much force to make him scream in pain. I hurt his back, I don't feel too bad, he was a loudmouthed prick.
Back to the question, Pain-wise I would have to go with Goldberg's spear. Lame move-wise, Cena's FU.

I'm pretty sure that most guys get a little skittish around pile drivers, especially with the amount of accidents and neck injuries. i know there's a certain level of trust, but it's still gotta be somewhat freaky

With the exception of the headbutt anything off the top is scary as hell. I usually closed my eyes and counted 3 seconds to embrace for impact. Even though thats the last thing you are supposed to do in the ring.
And I feared the back body drop. dont ask me why but being upside down with no control makes me freak out bad.

Nobody's mentioned the Flipping Piledriver (aka Candian Destroyer)? I would piss my pants if I knew I had to take that move.

I dunno about that one, i mean, yeah, looks scary as all get out, but i think that it would be used only on people confident enough to take it, and that management would only allow it done to veteran preformers, maybe i'm wrong, i dunno, don't watch TNA all that much

i would probably "flip out"about the canadian destroyer as well buit i would have to work with the guy a little bit to get some fatin in him for the move. Hell if i can trust a guy to take the vertebreaker from him surely i can get enough faith to take that.

But, after seeing it a few hundred times, I think more reliability is on the person taking it than is often given. The person on the receiving end has to move on the way down just to make sure they don't become a human lawn dart. There's a huge timing element involved, and that's on both people, but still pretty scary.

its not really a moving thing as just ducking your head in time. Which i am proud to say i had perfect enough to scare the booker into saying i couldnt take in his ring anymore
