I'ld say what happened to Mideon.
I remember a shell of his former self talking to JR and King saying something like 'He's coming' or 'Save me'. Not sure which. Then the APA come out and drag him away.
Next week, we see him in a bizarre old abandened warehouse weak, defenseless, scared and possibly bloody.

Dude, he's an indestructible zombie who commands lighting, thunder, earthquakes, can make it rain blood, has attempted to kill abotu a dozen people in the last, like, 5 years alone on TV, and has a pechant for kidnapping and crucifying people solely becuase he can. He's ALL creepy.

If I remember correctly, didn't The Undertaker tear through the bottom of the ring and pull someone under? I think I remember seeing that, but I was pretty young. He did. At an In Your House event. Bret/Diesel in a cage. He ripped through the bottom of the ring to prevent Nash from escaping and allowing Bret to get out safely. It was in retaliation to the run-in Nash did in the title match at that year's RR.

Undertaker is at his creepiest when he's not summoning lightning, but actually doing something more...occult.
So, quite easily, he got really creepy when he had his Ministry appear outside what was allegedly the McMahon house, all of them smirking smugly and all of them clad in dark, his voice-over making very cold threats ("Or maybe I'll just torture her.") and sending Vince and Shane into a panic ringing up the phone backstage, trying to call the cops or tell Stephanie that he was outside. While obviously fantastical, it actually seemed like something that could happen in real life, and that he would menace and prowl after a young woman, somehow even obtaining a stuffed animal from her childhood and igniting it in front of her father, really did chill the spine. He also sported the look that is my favorite of all Undertaker appearances, slicked-back hair almost to a widow's peak, a sharp goatee, often in a druid's robe- it all seemed to emphasize his skull, which was surprisingly artistic.
'Course, we got Stephanie calling him "Mark" out of all that and hugging Austin when he came to save her, sooo there were some mildly annoying moments in all that...but damned if the Undertaker wasn't at his most chilling during that angle. God I miss Ministry 'Taker.

I literally had nightmares when I was a kid after I saw the 94 Royal Rumble when Undertaker "died" and his soul left the arena. As cheesy as it is now, back then it was terrifying. I know it seems cheesy now, but it still is one of my favorite Undertaker moments Agreed. That STILL creeps the crap out of me. I also remember when Orton was feuding with UT, and they got the Undertaker lookalike in the casket (EXCELLENT job), and vice versa.

My little mark self cried when Kane set the casket on fire with Taker in it after the Taker/HBK Casket Match. That was at the '98 Rumble i think?

One thing I didn't like from Satanic Undertaker was when he occasionally had the tear drop. From far away it looked like a large mole. I loved the Ministry and Undertaker during that time. I did not like, however, when they joined with the Corporate and McMahon was eventually the "Higher Power".

Nothing really ever creeped me out, but his hand bursting out of the dirt after the end of the first Buried Alive match v Mankind was cool.

The pain is still there, a potentially great angle wasted

I always wondered how they were able to "bury" Taker in this match. Did he have oxygen on him, did he sneak out of the grave somehow...how exactly did they pull this off? I've always wanted to know. As a side note, I begged my mom to order this PPV, b/c the WWF said it was a "non-sanctioned" match, so I assumed it would never be shown again. Imagine my surprise when it came out on video 3 months later. ;D

When he hung the Big Bossman at Wrestlemania 15. My friend and I thought the creative team took it a little too far.

Another thing that wasn't really creepy but had me kind of concerned, was when I think the storyline was that Paul Bearer recently revealed that Bearer was Undertaker and Kane's father, and he was having an interview at his home where he was gloating about it, he was gloating about something anyways when the Undertaker came to his house and stomped a mudhole in him....I don't know, something about watching poor vulnerable pudgy men getting their butt whipped brings a tear to my eye.

When Mideon was sacrificed. It was freaky for me, the 12 year old mark. I've worked in theater, so I later learned that knife was a fake and it has a built in blood hole at the razor end so it looks real (I guess that's what they used. More than likely it was.) but for years I beleived he actually cut himself. In early 99 NOTHING was more evil than the Undertaker. Ministry Taker was scary as hell.

Probably when Diesel destroyed his casket. As a kid I remember thinking he was hiding in my shower while I was taking a piss. I don't know why. Kane was a lot scarier destroying Dude Love and others in his debut. Freaked me out, atleast.

On a Monday Night Raw in 1996, I think it was leading up to Wrestlemania 12, Diesel opened a casket at ringside and discovered himself inside of it! It looked EXACTLY like Kevin Nash, and it startled me. Granted, I was ten years old at the time, but still...

Talking about more recent angles, him crucifying Orlando Jordan during the JBL feud was pretty bad-ass.

Creepiest Taker moment has to be when someone posted a video of him on Smackdown with the Christie Hemme ring entrance
