If you've played IWBTG be honest, this shocked the hell out of you the first time it happened.

Do you actually have to dodge the glass being thrown at you? Man, even the cutscenes in this game can kill you...

I like the one game over in IWBTG where you see a Windows error message that proceeds to fall down and crush you if you fail to dodge. The fun thing is that on Windows XP the game shows that exact error message whenever it crashes, so the trap can really catch you off guard.
Edit: You can also die during the end credits. No, I haven't made it that far, but I've seen people LP this thing.

Cloud's LP of IWBTG is the best one imo. He has about 10 minutes of him going bats*** crazy on MechaBirdo alone. Ah, f***ing MechaBirdo. I've never gotten past him (her? it?) for some reason, it should be one of the easier boss fights in the game but I just can't do it. At least I made it through the Ghosts & Goblins stages without losing all of my sanity... ...speaking of which, the single best part of UltraJMan's LP of IWBTG is the Ghosts & Goblins section. I didn't watch Cloud's LP for too long so I don't know if he had an equally epic meltdown with the spike traps, but I assume he did because that part of the game is absolutely ridiculous.

In MGS4, if you die in the later parts of the game, you get the standard game over screen, but if you put the cursor on "Exit", the words shift to "Exist" & you hear Liquid say "Brother! It's not over! Not yet!" & it puts the cursor back on continue.

The fake memory card corruption in Eternal Darkness: Sanity's Requiem for GameCube. Also, a classic, though not "shocking" per se:

The Trauma Center series has some really melodramatic game over screens, especially since you will be seeing them a lot.
