Who on the WWE roster currently has one strike or more against them?
I came up with:
Dolph Ziggler (1 violation) John Morrison (1 violation) Rey Mysterio (1 violation) Mike Chioda (1 violation) Darren Young (1 violation) Sin Cara Azul (1 violation) Booker T (1 violation) William Regal (2 violations)
Anyone I'm missing? Do Santino and Orton count, as I remember them being linked to the mass suspensions in 2007.

Orton has the one, and possibly a second. That one's iffy, but the one definitely counts - they just had him work without pay rather than suspend him.

According to pwpix.net (reliable source? idk) Booker T has had 2 suspensions. Both in 2007. His second was a 60 day suspension in August 2007, right before he left. I seem to remember hearing about this and a lot of people speculated that is why he left and that it was because of Marijuana.
Otherwise it looks like you have a complete list unless we're going to count FCW talent as well. I know developmental guys get wellnessed sometimes, but I have no idea how many of those guys are still there.

Doesn't Sheffield have Wellness Strikes? Just checked and apparently he does. He was suspended in 2006 for his first violation of the wellness policy.

Don't you get a clean slate if you leave and come back? Well technically anyway, I'm sure management would still hold something against you if you were wasting your second chance.

I don't think so but I can't remember if we've had any instances of a wrestler getting suspended for wellness, leaving the company, coming back, and then getting suspended again. I don't think that's happened. Jeff Hardy had something like that happen to him but the trouble he got into during his first run was before the wellness policy existed and I don't think he even got suspended he was just outright fired. Still, I think the wellness violations don't just go away when someone gets released. The terms of the wellness policy are out there somewhere, I remember seeing the complete policy a few times when they first started it.

You don't (or at least didn't) get wellnessed for weed, Brian Kendrick had like a dozen fines for weed before he got FEed and I doubt Kendrick had higher stock than Orton

I believe Orton's documentary stated he was suspended after he OD'd on meds in '06.

Undertaker and HHH were fined for a chairshot to the head. I don't know if it counts as a strike.

I remember reading back when they made Hardy champ that if someone does get 3, they are fired and gone for at least one year before they can be brought back. If brought back, they are on 2 strikes. As for getting FE'd with one or two strikes, if you return, you stay on whatever you were at when you left. Chris Masters for example was at two the time of his first FE'ing and when brought back, still had two against him I believe.

Orton's marijuana-related suspension was a behavioral suspension, not a Wellness suspension.
