How do they load someone under the ring? Do they have to set up pre-show and then sit there...
Or do they have some clever way of disguising them like a camera guy or something?

Nope. They just sit under there, usually with a monitor (like the ones at the announce table), so that they can catch their cue.

I think I heard one time they have a special mat and stuff under there, as well as a monitor to remind the guy when it's time to come out. For if the person was on the show before hand, I'm not sure. But disguised cameraperson could work. Both Shawn during his feud with Angle as well as Edge that one time were able to pull it off.

Wow, thats lame if they have to sit under there the entire show... Plus I would imagine at least an hour beforehand while everyone is coming in. Although I guess it isnt all bad if they get to watch the show during.

and I'm not sure, before Kanes entrance played did his lights go out?

Usually during commercial or something the lights will be low and a group of about 10 guys will come down to the ring to set up some stuff with the mat and all and hiding in the middle is the guy who's going under the ring.

I figured he went under there when the lights were off for Jericho. He would probably look odd if he was disguised as a camera man (being that he's huge).

So how did we get this far into the explanation without the story of Perfect s***ting under the ring while he was waiting there?
Someone was with him too, like DDP maybe?

This. If u ever been to a live show, almost between every match is a promo or commercial played with the lights dimmed or blacked out. Many ring crew members come out, some with black hoods, caps... some crouching "running cords". Any one of those could slip under the ring, as many arent paying attention.

My guess: when the lights went out for Jericho's entrance, Kane rushed under the ring since it was the segment before his anyways.

I remember reading live reports from when Taker and Kane did that switching between the ramps and in the ring bit. Looked cool on TV, live you could just see them casually walk in and out out the ring.
Between the slew of guys, it being dark, them blasting stuff on the Tron, who the hell is keeping track of guys who crawled under the ring for some reason and didn't come back out?

I went to a Smackdown taping in '04 where they did the whole play something on the titantron as the lights go out bit. It was for Undertaker.
Also, the Mr. Perfect story...there were other members of the NWO hiding under the ring with him. It was around the time Warrior was in WCW. Apparently, Hennig took a dump under the ring in a bucket and it caused Scott Norton to puke.

I want someone to go and look for a table under the ring during a match, only to find some random wrestler sitting there.

They go through the same tunnel DX used during the Little People's Court segment.

I went to a Smackdown taping once, and about six guys rolled a big box down to the ring in between segments. I forget who was under the ring, but that's how they did it that time.

I've always wanted the lights to come back on and the guy hasn't got fully under yet so when the lights come on you see Kane or whoever crouched down, lifting the ring apron up and looking sheepish.

I always thought that they could utilise certain features of the arena they're in. The O2 Arena, for example, has a platform that can lower from the floor to underneath. They could theoretically just put the ring over that platform.

Guys I think you've missed it completely here.
Kane LIVES IN HELL. Hell happens to be directly beneath the city of Memphis.

As I recall, that's how Big Show got under there for his debut at St. Valentine's Day Massacre. Large box for the large Big Show who snuck under while they were assembling the cage.

One lesser example was that one Smackdown Brock vs. Angle match that happened before WrestleMania XIX where Kurt and Eric pulled the switcharoo.
