In X-Men Legends, I didn't realise I could throw characters with other characters until I used Colossus to send Wolverine into a spike pit, killing him instantly. Aww, it always works in the comics:

Been many a time ... When I have poorly thrown a grenade or other explosive that has resulted in either my own or a friendly NPCs death ... Or both.
What game? Ummm GTA, Fallout, Mass Effect, Red Dead, Medal of Honour, Call Of Duty.... And so on and so on and so on.

I have a bad habit of sending units into battle in strategy games without thinking of important details.
In Super Robot Wars, I often forget to change equipment. Water map? Will totally forget to equip a flight module or something to help not get bogged down in the ocean.
Fire Emblem? Forget my bowman's iron bow only has like 4 uses left.
Fianl Fantasy Tactics Advance? Changing classes without equipping a new secondary skill if I was already using the one that I changed to, making it the primary and just leaving me with a blank.

Any non-ocd rpg fan knows the horror of the playing for multiple hours without saving only to be felled by some jobber random encounter because you thought you were too cool for school.
The real bitch of it is at some point during this process, you looked at the save crystal/inn/menu screen and thought about it.
In an action game, whatever. You maybe watch a cutscene, bang bang, pow pow and you're back shortly.
If you allow it to happen when playing an rpg, more often than not it seems to be in a situation to where you have to sit through waves of dialogue or do a particularly tedious portion of the game somehow.

I... am genuinely confused about what the problem is here. And now for more SsnakeBite gaming fail: Playing Soul Calibur III, the opponent is near the edge of the arena. Naturally, the best course of action is to charge and go for a flying kick attack! So yeah, they dodge and I end up falling out of the ring like a jackass. Playing Skyrim. Pay 1000 coins for a horse (half my money at that point), all happy, go ahead for some adventuring all proud of my newly-purchased horsey, just chilling down a narrow road. Suddenly dragon. My horse is killed within a minute of buying it... And what may be my finest hour: my very first kill on Counter-Strike Source. I had been playing for half an hour and the fact that I am indeed talking about my FIRST kill should tell you how well I was doing. I kept getting shot by much more experienced players and was growing frustrated. Then at some point, I see a corridor where a smoke grenade was thrown. Showing the first sign of intelligence in that day, I realized there was a good chance that enemies were waiting for me behind it, so I decide to instead wait in front of it, aiming and getting ready to shoot the instant I heard someone walk towards the cloud, with them unable to tell I was there due to said cloud. The plan worked perfectly, the only way it could have gone better would have been if it had been an enemy I shot.

Constantly forgetting that grenades & concussion bombs do not work in the first 10 seconds of a CoD online game. I'm usually the first guy killed because of that.

I lost count of the amount of times I had to reload my game of Skyrim because my companion decided it was the perfect time to walk right in front of me when I was in mid-swing.

It took me approximately 20 minutes to figure out how to leave my house in Pokemon Red, and I started to irrationally worry that this was the entire game (My excuse is that I was 7).
In happier news, it went on to become my favourite game of all time. Just as well that I eventually got out.

With Mass Effect, the grenade button is the 'put away weapon' button in the sequel so going back and playing it frequently shows me throwing a random grenade when I want to put my weapon away.

I'm pretty bad at reloading in Online shooters. I do it pretty much constantly after every encounter with enemies, and I'll get killed alot whilst doing it.
But you can guarantee the one time I forget to, I'll bump into an enemy that I could easily kill, then realise I've run out of bullets and get murdered. Also, getting the controls mixed up between different games, for example, first few days playing Battlefield 4, I found myself stabbing the air when I wanted to hide behind something, getting me killed, or going to stab someone and ending up crouching behind them, which just looks like I'm sniffing their ass.

The first time I played Sonic the Hedgehog I kept jumping over the big flowers in Green Hill Zone because they looked spikey and I thought they would kill me.

I'm the master in Smash Brothers of accidentally doing a "dive attack" when an opponent is in the air under me and there's no platform to land on.

Never disturb the Cazadores. That is all. Yeah f*** that s*** Absolutely hated that in the game. Even when high level stats still tough as hell. Unfairly so I'd say, especially when they are in packs as they often are
