In wrestling, an obvious answer is Barry Darsow replacing Randy Colley as Smash.
Mark Ruffalo replacing Ed Norton worked better than I would have guessed for The Avengers. I didn't like the change from Terrence Howard to Don Cheadle in the Iron Man series.

Wendee Lee replacing Katerina Luciani as the voice of Alpha 6 in Power Rangers. Like most others the faux jive talk didn't do it for me.

Noel Neill replacing Phyllis Coates as Lois Lane on the Adventures of Superman (Neill had previously played Lane in the serials, but they were an entirely separate continuity). Eh, I just watched the whole series on DVD and I gotta say I'm a little iffy on this one. While Neill better personified how I would expect Lois to be, I think Coates had better chemistry with George Reeves. And while Noel Neill was a beautiful woman, she was really starting to show her age towards the end of the series.

Jefferson D'Arcy replacing Steve Rhodes on "Married With Children" Yup. The "Ted McGinley curse" had never made much sense to me, considering MWC lived so long with him in the cast.

OK, this one is probably the craziest one I've seen in a series of movies... then again, it's from a series of movies that no one is supposed to take seriously.
In the 1991 action flick "Do Or Die" (#6 of the "Girls With Guns" series of movies, written and directed by Andy Sidaris), Pat "Mr. Miyagi" Morita played Kane, the Asian crime lord behind a series of attacks on the main characters, a covert government police agency (who all happen to be former Playboy models). For those who don't know, Pat Morita is an Asian-American who was 59 at the time.
In the following three movies of the series ("Hard Hunted", "Fit to Kill" and "Enemy Gold"), the character Kane returns, but now being played by Geoffrey "R.J." Moore, a then-26 year old British actor best known as being the son of Roger "James Bond" Moore.
They even re-shot one of the scenes to establish that it was, in fact, the same character.

Christopher Sabat as the voice of Vegeta. I really like Brian Drummond, but Sabat is who I think of with Vegeta, it's impossible to go back without it being a little weird.

I honestly thought Alpha 6 was voiced by one person the whole time. Unpopular opinion, but I preferred the "Yo-yo-yo" stuff. I thought it helped differentiate Alpha 6 from Alpha 5.

Kevin Murphy taking over the role of Tom Servo from Josh Weinstein on Mystery Science Theater 3000. Winner. (Also, Amy Poeller season 2 Parks and Rec replacing Amy Poeller Season 1.) Steve from Married With Children is awesome (for me). Timing was great so the big laugh was always well worth the wait. Chuckles build up for the out-of-character moment of a-ha-ha-ha-ha-ha. Kept it in the pocket. Jefferson is just freaking screaming jokes and beating audience over the head with perversion and forced laughs. Maybe a guy people would want to hang out with over Steve, I get that. But in terms of character ... pretty much underdeveloped for me. Just a tool trying to be a rich Al or have the best of both worlds. Dial it back a notch, dude.

Sarah Drew coming in as the new voice of Stacy in Daria halfway through Season 1. The character slowly became one of the most popular in the whole show, even though she was nothing more than 4th banana in the Fashion Club, to the point where she actually got the B-plots in some episodes of the final season.

I've picked it up from the replies, but some insight from the actual headline would be nice. 2 Darrens is a reference to Bewitched where they replaced Dick York with Dick Sargent at one point... yet they were supposed to be the same guy and no one mentioned as far as I am aware of.
