Diamond Dallas Page: Was in a faction with Kanyon and Bam Bam and won titles with each. Then he tagged with Kevin Nash.
Chuck Palumbo's been mentioned as a tag partner with Billy Gunn, but he was in The Perfect Event with Shawn Stasiak in WCW, he won a title with Sean O'Haire when they were Natural Born Thrillers, after Billy and Chuck he was in the Full Blooded Italians.

This seems to happen all the time during the Attitude and post-Attitude era's. A promotion has a fairly successful tag team, thinks one or both members of said team should have a singles push so they break the tag team up. The singles push doesn't work out so they're placed in a makeshift tag team with someone else who isn't doing anything at the moment. That new tag team winds up getting over and the cycle repeats itself.

-Johnny Hotbody & Chris Michaels (Suicide Blondes stable, ECW) -"Prime Time" Brian Lee (SMW) -Bodydonna Zip (Tom Pritchard, WWF) -Lance Storm (ECW)
Won tag belts in every version of these teams. Reliable and multifaceted.

Tenzan is probably the most successful regarding staying with his partners, he's was apart of the one of the top teams of the 90s in Japan with Chono and afterwards in the 00s and now is in another top team with Kojima and teamed and won with Osamu Nishimura as well.

So many come to mind. This could be a massive list really.
Samu replaced Sika in the Wild Samoans in the early 80s before forming The Samoan Swat Team/Head Shrinkers.
Has anyone mentioned Rick Steiner yet? The Varsity Club and The Steiner Brothers. Could also mention Mike Rotunda there for the Varsity Club and Money Inc.
New Jack was in The Gangstas and The Gangstanators, while Kronus was in The Eliminators and The Gangstanators.
Scotty Too Hotty and Albert teamed (the Hip Hop Hippo days) after their respective time in Too Cool and T&A. Not sure if they ever had an official team name.
Paul London had runs as partners of both Billy Kidman and Brian Kendrick.
Nikolai Volkoff first teamed with The Iron Sheik before forming The Bolsheviks with Boris Zukhov.
Davey Boy Smith had teams with The Dynamite Kid (The British Bulldogs obviously), Lex Luger (The Allied Powers), and Owen Hart. Owen Hart, meanwhile, had teams with Koko B. Ware (High Energy), Bret Hart, Yokozuna, and The British Bulldog.
Mosh/Chaz went from The Head Bangers to Lo Down.
Christian has teamed with Edge, Lance Storm, Chris Jericho, and Tyson Tomko.
