Wii Motion Plus. Mostly because the original controller is an unusable pile of dick and there should have been a class action against them for selling it in that state.

PS1 memory card were sold separate if memory (pun intended) serves me right.

The N64 memory expansion was must-have. Without it, you couldn't play Donkey Kong 64, Majora's Mask or most of Perfect Dark. Yes, it was 'must have,' but only for a small number of games that came out near the end of the console's lifespan. If you wanted to play early N64 titles like Mario 64 or Shadows of the Empire, having it made no difference.

Has there ever been a really succesful console add on? ... The closest thing I can think of is N64 memory expansion/rumble pak, but those are really small additions. To answer your question, I'd say no. Like yourself, I think the closest thing to a 'must have' add-on would be memory cards or the N64's rumble pack. However, if you were like me in the sense that you had a Sega Genesis / Megadrive and loved playing 2D fighting games, a 6-button controller (as opposed to the outdated 3-button one that was packaged with the console) was practically a must-have peripheral.

The dual shock controller for the ps1. (Released later in life, they became the only controller viable for the console)
The nunchuck for the Wiimote.
Pro controller for the Wii U.
HDD for the 360. (They used memory cards at first)

This, and the Gamecube version. And its worth mentioning that the PS4/Vita compatibility was the Switch before there was a switch.
