I was just trying to think of some shows that ran for quite a while and ended up having several different themes/opening titles. I came up with Everybody Loves Raymond, which had four (First was Ray talking as his family floated around, second was Ray hiding from his parents and brother, third was the family playing football in a park and the final one was a clip compilation set to the song 'Jungle Love'). And then there was Sabrina the Teenage Witch with three, which reflected Sabrina growing up/going to college etc.: www.youtube.com/watch?v=wL6pCdpNPNIAny others? There was six. You forgot the pilot where Ray gets stuck inside of a playset he was building & the conveyor montage. Nah I didn't forget, I just didn't include those because they're too similar to the first one (same music and similar concept).

Boy Meets World had a new intro for the first six seasons until they repeated the same opening for Season 7.
Season 1 was the most traditional, names attached to the actor's photo.
Season 2: Cartoon Cory get run over by the globe.
Season 3: Cory, Shawn and Eric at Chubby's playing pool.
Season 4: Cory, Shawn, Topanga ride with Eric and watch clips from previous seasons.
Season 5: The "Friends" style opening with new cast member Jack.
Season 6: The "Friends" style opening with new cast members Angela and Rachel.
Season 7: Repeat Season 6.

Degrassi:TNG has a bunch of different ones, depends on cast changes usually. The theme changed too

I liked Fringe's intros. They weren't different just because it was a new season; often, the intros were different because you were in a different place in time, if not a different dimension altogether.
And, of course, Community.

New Girl has had a few based on length. There was a more recent change as well. Based on research, the Pilot had a different one as well.
Scrubs as well. They had the standard (I'm no super man one), a super weird longer one for a few episodes, then a shorter one.

Agents of SHIELD's intro is basically a 5-second title card, but they have changed it up on occasion, notably in the fourth season to represent different story arcs.

I think animes are all contractually obligated to change their theme every 12-24 episodes or so.
