Wizardry for NES is up there, as is X-Men. The only game for NES I returned. I played Back to the Future and that's pretty bad as well. Ghostbusters is hot garbage but it's a pretty easy game to play. Werewolf is a game that is outright broken if you turn into a human in the first level at the wrong point.
There's an influx of awful SNES games, but the funny thing is a lot of them would be good on the NES. One game that stands out to me is Michael Jordan Chaos in the Windy City.

I dunno I played a lot of the ones that are usually listed as Worst ever.
Like E.T. and Superman...
of those two I'd put Superman as far worse, and despite what most people said it wasn't just the rings... that's only the icing on the shitcake that is that game.
There's also terrible fighting stages, and terrible stopping speeding car stages... and terrible controls... and it really doesn't have like anything I'd put as a positive...
edit: Deadly Towers is up there too... X-men for the NES... Wrestlemania 21 was a disaster, especially after Raw 2 which while it was REALLY flawed was also at least fun.
I'd also throw Wrestlemania x8 for Gamecube on the list of bad wrestling games... it was horribly rushed I remember the models being really shiny... a bunch of people not actually having their music and having cheap knockoffs instead... and I don't even think it had a season/story mode.

Especially the SNES version. As bad as the Arcade and Genesis versions were at least they were at least playable and had that 90's cheese factor. The Super Nintendo version on the other hand had poor controls, horrid graphics, missing fighters, no regenerating health between levels and worst of all NO CONTINUES! Button mash and pray. No skill, no strategy and no fun. This game (SNES) was horrible. I remember renting it and brining it back after 15 minutes.

The first console game I had was Pong. (Yes, I am that old). Can you imagine playing that for hours on end? Well, I did.
Hitchiker's Guide to the Galaxy (1984) - Very early game. Entirely dialogue based. I could get as far as the Heart of Gold and then....nothing. Nothing I did hot me any farther. There were no websites or message boards to ask for help. You just tried different things until you got something right or gave up. I gave up.
Conquests of Camalot (1989) - a slow paced, King Arthur based game full of generally easy tasks and riddles. Except for the joust against The Black Knight. That was freaking impossible. My roommate and I fought that over and over. Two or three hours a night for over a week. When we finally got past it, the rest of the game was just boring and repetitive.

Absoloutley nothing redeeming thing about it not a single cool boss , level or even piece of music.
And the less said about rings the better.

Rise of the robots While I'm sure there's worse in the world of shovelware or early Atari (Never played it), But ROTR was a AAA title that was hyped to the heavens and ended up being a giant piece of shit I had a demo for Rise of the Robots 2 that came with a gamepad, The menus and cinematics were cool, but the actual game play was shite! I can only imagine how bad the first one was.

The first 2 are among the best ever. 3 was like Russo booked it.

Every time. My dad was obsessed with Zork style game I'd say the worst game from my childhood was "The adventures of Bayou Billy" on the nes. Poor man's double dragon. Absolutely terrible.

I’ll add to the Superman 64 chorus, but I have another one: I remember being 8 or 9 or however old I was and I was at Blockbuster on a Friday night and I was looking through games to rent. An employee walks over and says, “Hey, do you like The Simpsons? We just got a new game in” and he hands me Bart vs. The Space Mutants. Well, shit. I had to rent that because I was obsessed with The Simpsons. That game, folks, was not good. It was not good at all. I distinctly remember not making it out of the first level. Now, not every Simpsons game was bad. The next game, Bart vs. The World was okay, Simpsons Arcade Game was a lot of fun, I liked Bart’s Nightmare and Simpsons: Hit and Run was a classic. Bart vs. The Space Mutants was not...not even close. I really like Bart vs the Space Mutants, will still give it a play every now and then. I always liked the little things in it that you can do (like prank calling Moe in the beginning of the game) My vote goes to Superman 64, absolutely horrible game.
