Saw one article that said the selecting the message to send was a drop down menu with essentially "Missile Launch - Test" and Missile Launch - Real" with either no confirmation or a very generic confirmation (not one that says "ARE YOU SURE YOU WANT TO SAY THIS IS NOT A DRILL")
One would think the person who did this is super mega fired, but there was also an article which said they were just re-assigned.
