Kane's body was completely covered in burns. "but, I got better", because that's how scars from burns work. I think it was implied that his scars were mostly in his head. Yeah once his mask was removed they said they were mental.

That just reminds me that their attempt at Kane's "burnt" face just looked like he was covered in soot, like a cartoon bomb went off in his face. Seriously, did no one in the WWE makeup department know what burn scars actually look like?

Back when he first showed up their reasoning for his full body suit was that he was burned all over. Then bit by bit they removed bits of his costume and at some point they just kinda handwaved with he got better or something.
Edit: just saw Platypus's response as well. I must have missed them outright saying it was in his head. I thought it was just them kinda ignoring continuity.

More specifically with Bubba Ray, he was a stuttering hillbilly who lost the stutter and Southern accent became a New York accent. He then joins a new company and develops the stutter and Southern accent again, only to lose them both later.

The spelling of his name (either "Buh-Buh Ray" or "Bubba Ray") seemed to fluctuate over the years. He entered the WWF as the former and gradually became the latter.

The Usos go from talking about how "we all up in your schools, your universities, your law firms", sidestep to babyfaces embracing their culture, to "the Uso penitentiary". And Tamina...is still employed. So she got that going for her. (She used to be the daughter of some HOFer...can't recall who it was. )

on the subject of Kane's scars, didn't somebody get to see the scars and they acted like it was the most disgusting thing they ever saw, feel like it might have been a member of DX,
it's a pretty hazy memory

Zeb Colter's character for the most part was a subtly xenophobic old man who hated illegal immigrants. When he debuted, he was a straight up racist who hated Mexicans.
