In the Axis Chemical Plant. Following Jack shooting Eckart, Jack tries to shoot Batman, Batman deflects the bullet with his arms and then it ricochets into his face which causes him to fall over the railing. At least that's how I understood that scene. And IIRC the bullet pierced him across the face / thru both cheeks, creating scars that the chemicals would've then warped into that smile. That would make the most sense really.

Batman '89 is probably the 1st time anyone ever actually tried to explain his transformation physically, I think. The skin and hair color changing doesn't really need much rationale, but given that their makeup was going to be so pronounced and different they made a point to explain how his face got like that.

It will always hold a place in my heart like it does for a lot of people, but I can take off the rose tinted glasses to see the bad along with the good about it. It's by no means a perfect movie, or even a perfect Batman movie. There's definitely some of Burton's lack of familiarity with the source material that shows through and just some of the generally strange quirks and ticks he has as a director that seep into a lot of his work. It's like that person that may not be necessarily the best person in the world, but is so darn charming and charismatic that you're able to overlook and forgive a lot of their flaws. Which is more than I can say for some Batman adaptations. One of my biggest problems is... well how SMALL Gotham is in this and Returns, it's really obvious it was a soundstage.

Absolutely love the movie. Pretty well aware of its faults (really being a plotless sequence of a bunch of run-ins with a villain who has little to do with the source material is why I dislike the first Spider-Man and you could say the exact same thing about this one) but still find it immensely watchable.

I was 9 so did not see it in cinema sadly (but on VHS , same with 007 LTK)
But i clearly remember how big a deal this movie was back then , those Batman/Joker candy heads , licorice candy , the metal pins

Then again "never rub another man's rhubarb" ain't exactly an expression outside of Jack's Joker either. I think he was just having fun saying whatever silly shit popped in his head.
