In Hellraiser, Pinhead was simply named "Lead Cenobite". But fans dubbed him Pinhead and the name eventually became his official one.

The Clone Thor from Civil War was essentially named "Clor" by fans, and has since been semi-normalized in the comics by other characters also calling him that.
Pretty sure Blue n' Gold, the name of the pairing of Blue Beetle and Booster Gold, was a fan created moniker too.
And reaching into reality a smidge - people know the writer as Peter David, but online comic fans of the '90s and '00s will forever say "Oh yeah..... PAD." (His initials, which was part of his online handle in past years)

The whole cast of Ace Attorney Investigations 2, since the game was never localized. Thus, the names given to them in the fan translation are effectively their English names despite not really being canon.

Clive Barker also hates the name enough that in one of the hellraiser books he had Pinhead kill someone for calling him pinhead.

Comic Book Guy’s real name is Jeff Albertson. No one ever calls him this. Well, that was his name way after he had already been called Comic book guy for years. He was never referred to as Jeff Albertson before that.

Phantom of Krankor has usually been referred to by the name Krankor, when that was the name of the planet.
Kondo from "Gamera vs. Guiron" is forever, at least for the MST crowd, known as Cornjob.
