Dick Jones
He thought it was a good idea to give ED-209 live ammo, then forgot that Robocop could record things.

Dune, Terminator and the Matrix All involve humans foolishly creating AI that they eventually and inevitably lost control over. Dune? Bit late for the characters in that series surely. The AI gone rogue is in the distant history of that story.

Immortan Joe should have just turned on the water taps and taxed everyone for the privilege.

The AI gone rogue is in the distant history of that story. And is actually up for debate if that is what Frank Herbert meant with the events of the Butlerian Jihad. Granted, his son's books claim the rogue AI in the prequels and sequels, but his son is a dipshit and those books suck ass.

The Last Kingdom. Basically everything that happens. There's a long list of times there's been some sort of solution that could work for everyone only for one person to blow it up, usually for a relatively dumb reason.

Vader could've just force pulled the guy with the Death Star plans towards him.

Not quite the same, but most of the hero versus hero 'event' books could have been avoided if they showed a lick of sense (I'm thinking Civil War and Avengers vs X-Men especially) Avengers vs X-men required multiple people to act completely out of character and ignore all their combined history. I could probably write an essay on why the initial conflict made no sense. It basically just came down to Bendis pulling a Moffatt. It makes no sense but the plot demands it, so it's happening and just ignore any continuity (or really all continuity) that disagrees with the plot.
Civil War at least had an intriguing start, though it did become a bit overblown and in my mind turn Tony Stark and Reed Richards far worse than many of the super-villains they claim to be against.
But yeah, I definitely got to a point with Marvel crossovers where I felt like "do you remember when Marvel heroes fought Marvel villains in cross overs? Pepperidge Farms remembers".

If the power wasn't shut off by dickless over there, New York would be just fine and the mayor would have saved the lives of many registered voters. Is this true?!

In Pet Sematary, the guy was obviously grief stricken about losing his son and doing anything to bring him back, but he got SO MANY warnings that it was not going to go well. And then he did it again with his wife, thinking that it would be different since she had only been dead a short time, despite nobody ever telling him that that was how it worked.

I think there was a mention that the Wendigo wants people to use the cursed graveyard, so there's like a subtle mental pull towards the idea.
