Jericho had 'Break the Walls' for his entire WWE career, didn't he? 1999-2017ish without a change must be pretty unusual.

Did Jim Duggan have just the one theme through his whole run, or did he have Pictures at Exhibition during his time as King?
Jerry Lawler only ever had "Pictures at Exhibition" too, right?
As Waters mentioned earlier Randy Savage only ever had the classic Pomp and Circumstance through all his character iterations from 85-94 if I remember correctly.
Did Mr Perfect, Dusty Rhodes or Ted Dibiase ever have different short lived themes?
Mickie James almost fit this, but then I remembered her Royal Rumble entry to Hardcore Country.

There was an early version of this theme with different lyrics, it also got remixed 3/4 times with different instrumentals even though it's essentially the same song. Kurt Angle. They did alter his theme, but it was the same music, just rearranged so they couldnt have the You Suck chant. This was a whole new composition really, just based on his original.

Jerry Lawler only ever had "Pictures at Exhibition" too, right? Lawler VERY briefly had a different theme, maybe like a week or two of in ring, before switching back. Yet that alternate theme made it to WWE 12 (I think the 2K channel is OK to embed...)

I thought he only had this in the game when they couldn't license the Pictures at Exhibition recording. Had no idea he actually used it.

Yeah, the theme was called "Kingdom Come" and used very briefly in the build to WrestleMania. Just to have live action proof, here is an entrance on Raw with it.

Owens technically had a slightly sped up theme for a short time before they went back to the original. Searching ‘Kevin Owens Fight V2’ on YouTube yields some results from 2015, including an NXT entrance from when he was champ.

Ultimate Warrior
His first match as "Dingo Warrior" used a different song, but that was a televised house show for the local area.

Didn't Mickie James use that Toni Basil "Hey Mickey!" knockoff theme her entire WWE run? I'm not counting the one time she went into the Rumble and used "Hardcore Country" because she did it as a member of the Impact roster.

Don’t know if this counts but for a very brief time in 1995, their theme started with them saying: “We want hongi.” Hongi I guess was them rubbing their noses together. One person said hongi stood for: “We are on our last legs as a team.” Ha!

I remember downloading a very early Lesnar song with like a jackhammer sound or something back in the Kazaa days, so he definitely had another. But for the most part it has been the same one he still uses.
The list is definitely small unless someone is really new or wasn't around long enough to have theme upgrades/variations.
