Avril's 'Chop Suey' is terrible just because she obviously couldn't be arsed learning the lyrics and just thought "mrmrmrmrmrmrmrmrmr" were the actual words (just like everyone else who's heard the song). That said, I think it is a pisstake rather than a serious cover.
The worst song in the history of the world is 'Dancing In The Moonlight' by Toploader. That's a cover, so that wins.
Also of note, the Yo La Tengo album 'Yo La Tengo Is Murdering The Classics' lives up to its name.

You guys are close. I used to have a band and we had a cover of Tori Amos's Crucify that in hindsite, we slaughtered that song. I'm glad it never got recorded.
