My vote goes to Bat Out of Hell naturally... Fat guy sings his ass off is just a great concept...
My number 2 is probably American Idiot... I know I'll probably get killed for that, but some of the stuff on their, specifically Jesus of Suburbia, is some of my favorite stuff ever

I don't believe in concepts.
Concepts believe in me.

Machina: the machines of god.
But if the fact that the pumpkins are my favourite band ever counts as bias, then ill give a shout out to The rise and fall of Ziggy Stardust.

"The Rise and Fall of Ziggy Stardust and The Spiders from Mars" - David Bowie
It also doubles up as the best "album" album ever.

The Amory Wars...the collective name of all of Coheed's albums, which have a massive overarching concept. Amazing stuff, all of it.
My Chemical Romance's The Black Parade
Cursive's Domestica
Cursive's The Ugly Organ
A bunch of others...I just can't think of them off the top of my head at the moment.

This post wins the thread, particularly since it includes Animals and Final Cut, two vastly underrated albums.
