I remember one of the old Apter mags did a picture of Sting with the WWF title over his shoulder to go along with an article suggesting he might go there.

Speaking of Apter mags, back in 1995, in PWI, they took all the heights and weights of all the former WWE Champions, to see who would statistically be the real WWF Champion due to all the averages on physical statistics.
That man, was Nasty Boy Jerry Sags, and they had a shot of him with the WWF Championship.

Someone find that picture of Cactus Jack in 2000 - I know he's held the WWF title before, but as Mankind, not Cactus. It's kinda cool seeing Cactus as WHC.

I recall back around '92 when Ric Flair was doing his "Real World Champion" schtick on TV, I began to see photos in the magazines of him holding up a WWF Tag Team Title belt. Back when I was 11 it confused the hell out of me for a while. Later on I learned that during the time the WWF began distorting the image of the belt on TV, Flair had actually been carrying a tag title. Of course, the magazine photographers simply captured the still photos and they were never distorted in the magazines. I was just unable to find any photos on the net.
I remember one WCW Pay-Per View (I can't recall which one) where Curt Hennig was defending his US Title. But he came to the ring wearing the Cruiserweight title. I guess they lost the US belt.

Or the one with Piper holding the WWF title, looks like from 92 when he had a string of steel cage matches with Ric Flair on the house show circuit post-Royal Rumble.

Survivor Series 1991 when they had it blurred, it was funny since you could obviously tell it was one of the tag titles he was holding. I just wonder what the live audience was thinking.
