I am not sure but I thought I heard somewhere that the people doing the plugs company pays the places to have the person on. Just like a commercial you pay for hte air time.

In general, actors are paid a nominal sum to appear on talk shows as per Screen Actors Guild rules. Even though WWE doesn't deal with SAG, I would have to assume that they pay airfare, hotel and accommodations for the talent along with some kind of financial incentive.
I wouldn't think that Jeremy Piven would do the show for free, even if Kelly Kelly was a part of the gift basket.

I would image the USA Network meets the WWE halfway (Considering I remember reading that the whole Celebrity idea was somewhat their idea) on paying the guests

There's only one person who wound up not earning the amount of money that he spent that night: Seth Green (mainly because of him getting the rights for "Welcome to the Jungle" for his appearance).

They are getting to promote whatever their current thing is.....that should be payment enough imo.
But I imagine their food/hotel accomodations/transportation is covered as well.

I think it depends on the fact that the things they are saying might be written for them by creative.That would mean they are not actually appearing as themselves but as "characters" of themselves.Since aside from the Legends,who have contracts with WWE, most of the hosts are members of SAG so paying them would get SAG involved.I think ask 411mania.com my have talked about it better then I did but i don't feel like looking through old editions of it right now.

I guess it depends on who it is. Ted Dibiase might get a slight bonus (he's still their road agent after all) or he might get anything. Dusty Rhodes might get his usual PPA amount.
Assuming for the non-wrestlers, they probably get their travel ahd hotel paid for, and they get to promote their product on RAW. I think that's about it.
