I'm flipping through the pages of the DVD-ROM, and saw one I wasn't familiar with, and don't recall it having been inducted.
It appears this did not actually get inducted and was only one of the definitions, but it was described by Mr. Reynolds as "the WCW Mystery Man's $1.98 Halloween costume, consisting of a plastic hockey mask, black felt, and duct tape".
Again, I don't recall this, but can someone please shed some light on this?

It was your basic 'mystery man attacks people' storyline with a dumb costume, and turned out to be Road Warrior Animal.
On his own.
In 2001.
So I'm sure you can see the crap elements of it.

So I'm sure you can see the crap elements of it. Since it was only listed as a definition, I would venture a guess that it was just the angle, but I didn't remember anything about that.

I remember that storyline all to well because i was at wcw when the mask man was reveled to be animal

Yeah this was a weird one. They played up a mystery man in the main event of Sin like it was going to be a major defector from the WWF. It was kind of exciting because smart fans knew there wasn't anyone's contract that would be up any time soon but honestly had no idea who it would be. He didn't show up until 4/5 the way through the match after being hyped for weeks, particularly in his association with the Magnificent Seven. He revealed himself to be Animal and well, yeah. I would have rather have it have been someone like Barry Windham at that point.

Don't forget, Animal made his appearance after Sid messed up his leg big time. I think they rushed everything along when it happened.

I remember a "masked man" angle at WCW in 1995 I believe.
and there was more than one!
and they were attacking hogan!
and during the first "uncensored", we find out that it was arn anderson, and then later another "masked man" came out and it was randy savage helping hogan.
and later on, like weeks later or whatever, another masked man began to emerge, and it turned out to be one of hogan's clique, brutus beefcake, leading to another one of brut-i's least memorable characters.
I might've gotten a few of the facts wrong there, but that's how I remember it.
it wasn't a hockey mask, it was a typical "wrestler mask", like dick beyer or rey mysterio, and they were dressed all in black.

The first masked man angle in WCW was in 1994. It was Brutus Beefcake (the "other" never revealed masked man was Bunkhouse Bunk so I've read). In 1995, Arn was under a mask (not sure if he ever attacked Hogan or not), but he came out tied up later and the masked man was now Savage (this ACTUALLY made sense as it showed Savage had gotten the better of Arn backstage and stole his clothes).

I think the 2001 Masked Man was originally going to be Rick Steiner, problem was everyone had already figured that out to the extent that the crowd was barking when the mystery man appeared, so they switched in Animal instead, even though that made even less sense.
And then Flair and Animal turned for no reason in a pre-taped segment that aired the following night on Nitro. SWERVE!

Thank you. I actually was thinking of a gimmick where Rick Steiner was attacked by a guy in a bumblebee costume and the next week the guy unmasked to be Rick Steiner. Have not seen only heard of it.

Oh yeah, there was yet another "Mystery Man" angle that involved Nash getting run over by a limo in summer of '99 with a mystery limo driver running around. It was rumored Dan Severn was going to be revealed as the mystery driver, but nothing came out of it.
