I usually tell people that I know it's fake but that the hazards are real. ...please, don't try this at home.

Them: "You know it's fake, don't you?" Me: "Do you watch movies?" Them: "Yeah, I do" Me: "Then STFU!!!" Exactly. It makes no sense for people to say that wrestling is fake when they watch TV shows and movies: Why do some people cry over scenes in a movie when it's fake? Why are people sad over someone's death (human being or animal) in a movie when no one actually died? Why do some people actually express concern over the fate of a character in a movie/TV show when it's fake? Why do some people when watching a movie actually yell at a character ("Don't go in there!"; "Don't do it!"—as if the characters can actually hear them and heed their warnings), or talk about how dumb something they did was when they're actors following a script? Take it up with the script writer. Why do people make such a fuss about violence in movies/television when there's no actual violence being committed? No one is being harmed, no one is being killed. Save it for the actual acts of violence being perpetrated daily. Etc. Etc. It's just stupidity, which can easily be pointed out.

A better question would be, "Why are we supposed to care?"

The thing that gets me the most when it comes to non-fans are the MMA purists/fanboys. I utterly despise those guys. I like (Though not a huge fan) and respect MMA, boxing, all legit contact sports. I know boxers and MMA fighters go through lots of training and such, so do wrestlers. Personally, I don't understand why people compare wrestling and MMA. They're completely different things. Also, I would give that argument about movies, TV shows, etc. Although I have seen some people say that wrestling shouldn't be compared or "Don't give me that argument". I've been a fan for almost 12 years. Even though I've had an on-again, off-again relationship with wrestling over the years, my interest never died to the point where I stopped liking it. Stopped caring for it, yes, but stopped loving it, hasn't happened and probably won't. I will say this: I've gone to stores and bought some wrestling DVDs and rarely do I ever get a trip from the clerk about liking that stuff. Only thing closest to it was a guy asking if the blood was real and I was buying "Bloodsport: ECW's Most Violent Matches" DVD. I told him it was and he said that he gets a little queasy when he sees blood.

There seems to be a mental block with some people as they just don't seem to grasp the concept that these guys are genuinely hurting themselves at times. I really wish it were all "fake," maybe we wouldn't have some many premature deaths.
EDIT: Take a cue from Foley "real life is faker than wrestling".

What amuses me is the people who bring this kind of thing up the most often can usually name wrestlers from a few years ago, and I don't just mean the Hogans, Austins, and Rocks of the world. They'll name people who they would only know so well if they had at least casually watched on a regular basis at some point.
But no, they're quick to point out how fake it is and how they don't understand how people could like it. But they know a bit about it themselves.
I don't care. I'll talk about wrestling, watch wrestling, wear my Fanfest shirts and talk about the autographs and photo-ops I've got, and if someone around doesn't want to hear it, they don't have to listen. Or better yet, they can go somewhere else and find someone else to talk to/eavesdrop on.

I always thought the people who got wrapped up in it being fake were hurt when they found out it was fake and if they couldn't enjoy wrestling anymore because of it then neither could anyone else. The funny thing is, I don't remember being made fun off for liking wrestling but that may have been due to where I lived, the age I was at the time, and the fact that wrestling was popular when I watched it the most.
