I was a bag of garbage in third grade. I wanted to be a she devil, but my cheap ass mom wanted me to be a bag of garbage. Mom pushed this "idea" from October 2 to October 31. Mom acted like she really put some effort in to my "costume", but the damn thing fell off twice and newspaper kept falling out of my leg holes. So, it looked like I was pooping all night. BTW this is why I love the Iron Man Target commercial.

Not even costumes but mistaken as ones...
One year I decided to walk down to the grocery (and I always stop at other stores on the way). Everyone kept saying "nice costume" and all I was wearing was the same Rustler western style jacket I bought at Wal-Mart. I was wearing either an Eagles cap or beanie and a pair of Pumas... and had worn that jacket around town a lot as it was my only jacket at that time.
Another time, my dad asked if I wanted to ride along to the store, so I just threw on a shirt. The clerk laughed and said "nice costume". I looked down to realize I had thrown on a St. Patricks style one I have that says "Kiss Me I'm Original" with a four leaf clover.

My aunt dressed me up as a mouse one year back in the 90s. I was embarrassed because it was made to look all cute and homemade. I wanted to be anything but a damned mouse.

My costume one year was Michael Myers but I got the wrong mask. I got a Mr Spock mask by mistake. Does not look like Michael Myers when you paint it white

The earliest Halloween costume I can remember was wearing a Fred Flintstone Mask. I could not see out of it at all and the rubber band was uncomfortable. The next year I actually didn't wear a costume or mask. My mom said if someone asked where my costume was I could always say I was a girl dressed up like a boy.
Not one of the worst costumes but pretty funny was one year I did the cut holes in a sheet and be a ghost. Well throughout the evening I started off as a ghost. As time went on though I looked like a shepherd from ancient times, and and eventually a white blob.

Going as Ghostface killer (that killer from Scream) only thing legit about the costume was the mask other than that I just wore a long sleeve black shirt, a trench coat and black pants since we couldn't find the right size costume to fit me but hey at least I got the mask. Bad Scream killer costume? Been there myself. Halloween 1999............... That was my costume one year, but my pants weren't even black.

Not me, but my friend. He showed up to a party in his normal clothes and an NWO tshirt. I asked what he had come as and he said he was one of the million guys that nobody remembers who joined the NWO. Hilarious! I think for me, it was the year I had a little purple and black mask and was at Cedar Point. I think the rest of my clothes were just normal jeans and a sweater or something. I was on a ride in the ghost house, and somebody inside the ride (like one of the ghouls) took my mask from me!! I was pretty mad about it. Especially when during the ride, he held it up and taunted me with it.

If I have to go for my worst, I was a yellow power ranger when I was 8 or 9.
