Sid's leg snapping like a dry twig is about as bad as they get.
Honorable mention to the Spike/La Resistance spot where they threw him out of the ring and towards a table - except that the only thing that hit the table was Spike's neck.
Another honorable mention goes to the Vince/Undertaker Buried Alive match from a while ago. Vince bladed on the first punch from Taker, and was spewing blood into the first three roads for the rest of the match. The blood would shoot out in time with his heartbeat. It was awful.

Two more.
TAKA being thrown from the Rumble and landing on his face, shattering his orbital socket and breaking pretty much every bone in his face.
Angle attempting to belly-to-belly Shane through the plate glass at the KOTR. He did it about six times to break two windows, and each time it didn't work Shane landed on his neck and shoulders. Cool spot, but ... god, that couldn't have improved Shane's health.

Raven hit the Evenflow on one of the Villanos and...and I haven't seen this in years so its a bit fuzzy, extended the man's neck farther than it was supposed to go. That 's the worst botch I can recall. How I remember it was, it was Raven and Stevie Richards against the Villanos. Stevie and Raven were trying to do a Dudley Death Drop type move, but they didn't time it right, and the Villano got injured. It was actually Raven and Kanyon. Kanyon set up for a sit out powerbomb and Raven grabbed Villano IV's head for a neckbreaker but Raven started too early (or Kanyon was late) and he dropped down on his head quite badly.

If I ever wrestled, I'd never do the SSP for this very reason. I'd be scared that I would only move vertically, and just kill myself.

sid's leg is indeed gross. sabu landing right on his head after being thrown by benoit makes me cringe as well.
