First, Cena needs to back to his Doctor of Thuganomics gimmick full-time. No heel turn. His career continues as normal, blah blah blah. Then, four weeks before Elimination Chamber, Mideon returns with a prediction.

Cena, the Doctor, is unnerved by this but continues into Elimination Chamber with a shot at the WWE title. Wacky angle hijinks ensue, making the Chamber match consist of Cena, Orton, Sheamus, Miz, R-Truth and the Doctor's new best friend, Al Wilson.
After a gruelling match, in which R-Truth decimates the Miz when he reveals he put the tune of "What's Up" in R-Truth's head when he was five years old, Cena emerges victorious. Until...

Al Wilson is still trapped in his chamber pod. And it's filling up with smark radiation, reverberating around the "glass" cell, which can kill even Cena. The Doctor is pissed. He's so close to being champion. He yells it's not fair, he could have done so much more and that he should leave Al Wilson to be torn apart mercilessly by the internet. Al begs Cena to leave him, but the Doctor (of Thuganomics) knows what he must do. He exchanges places with Al...
"I don't want to go..."
Cena, the Doctor's, body bursts into flames, his face obscured. When the flames fade, he's revealed to have transformed into, oh, let's say Zack Ryder.
